がん治療における感染予防・制御に関する患者の視点：がん患者とその近親者における知識および態度の調査★

背景
抗菌薬耐性率が高まるにつれて、がん治療における感染予防・制御策（IPC）の強化が必要となる可能性がある。臨床ガイドラインを作成する際には、患者の視点を取り入れることが重要である。
目的
ノルウェーのがん患者とその近親者を対象に、IPC に関する知識および態度を調べた。
方法
専門委員会での議論とパイロット研究を通じて、知識の記述の正誤を判断する 13 問と、態度に関する項目をリッカート尺度で判断する 40 問で構成される調査を作成し、2023 年 8 月 22 日に、がんの経験がある人々に送信した。平均正答率および態度スコアを報告した。
結果
回答者 551 例において、IPC に関連する知識の質問に対する平均正答率は、79％（95％信頼区間 78 ～ 80）であった。回答者においては手指衛生に関する知識が最も豊富であったが（99％、551 例中 546 例）、抗菌薬耐性の予防におけるその役割に関する知識は最も乏しかった（41％、551 例中 225 例）。IPC については、とくに患者側の責務としてテーマ内で強い支持が認められ、平均スコアは 4.83 であった。しかしながら、一部の特定の立ち入った IPC、例えば近親者との接触を減らすことなどに対しては、著しい抵抗が認められた。
結論
本調査では、ノルウェーのがん患者とその近親者における、IPC の重要性を裏付ける高水準の知識および態度が明らかになった。今後の IPC ガイドライン作成において、患者の視点を取り入れることが推奨される。
監訳者コメント：
癌患者は感染リスクが高く、発症した場合にも望ましくない結果をもたらすが、特に抗菌薬耐性は治療が限定されるため癌患者にとって迫りつつある脅威である。耐性菌による感染症の半数は施設内感染であり、拡大的かつ侵襲的な医療の導入により、その感染リスクは増加している。がんケアにおける感染対策を強化するためには医療従事者側だけではなく、患者の抗菌薬耐性の知識を増やす以外に、面会などの精神的な面と感染対策を比較考量し、患者側の視点も考慮した感染予防対策の導入が必要である。
