日本における手術部位感染症の疫学および予防★★

日本で医療関連感染制御の実践が広く認知されるようになったのは、1980 年代半ばに感染制御に重点的に取り組む学会が設立され、大規模な大学病院が感染制御部門の設置に着手し始めてからである。1990 年代後半に、学会は主に手術部位感染症（SSI）に取り組む全国的サーベイランスシステムを構築した。同時に、米国疾病対策センター（CDC）により公表された SSI 予防のためのガイドラインが 1999 年に改訂された。このガイドラインは日本語に翻訳され、日本の診療業務において広く参照されるようになった。それ以降、日本の疫学調査と予防実践はともに進展している。全 SSI 発生率は、2000 年代初期は約 10％であったが、2007 年までに 7％に低下し、2020 年にはさらに 5％に低下した。サーベイランスにより大規模な SSI データベースコホートが作成されたことで、さまざまな種類の手術による SSI リスク因子に関する調査が実施できるようになった。2010 年代半ばに、CDC の SSI 予防ガイドラインの改訂と世界保健機関による新たな SSI 予防ガイドラインが発表された。正常体温や消毒薬含浸縫合糸などのエビデンスに基づく SSI 予防のための新たな実践が推奨されており、日本の手術医療に適時導入されている。しかし、SSI 予防に有効であることが示されている手術医療および機器の多くは、公的医療保険による償還が認可されていないため、これまで日本の医療ではこれらの方策を広く使用することができなかった。

日本における手術部位感染症の疫学および予防が、国家プロジェクトとしてどのような動きがみられたのか、これに誰よりも深く関わってきた著者の解説である。今後を見据えて、将来的にどのような課題がありこれを改善すべきかについても述べている。
