Effect of participation in a surgical site infection surveillance programme on hospital performance in Japan: a retrospective study

背景

厚生労働省院内感染対策サーベイランス(JANIS)プログラムへの病院参加が、手術部位感染症(SSI)予防に及ぼす影響は不明である。

目的

JANIS プログラムへの参加が、病院の SSI 予防実績を向上させるか否かを明らかにすること。

方法

2013 年または 2014 年に JANIS プログラムの SSI 部門に参加した日本の急性期ケア病院において、本後向き介入前後研究を実施した。研究参加者は、2012 年から 2017 年に JANIS プログラム参加病院において SSI サーベイランスの対象となる手術を受けていた患者で構成された。介入の定義は、JANIS プログラムに参加 1 年後からフィードバックレポートを年 1 回受け取ることとした。介入前 1 年から介入後 3 年間の標準化感染比の変化を、以下の12 の手術手技について算出した。すなわち、虫垂切除術、肝切除術、心臓手術、胆嚢摘出術、結腸手術、帝王切開、脊椎固定術、長骨骨折の観血的整復術、幽門側胃切除術、胃全摘術、直腸手術、小腸手術である。ロジスティック回帰モデルを用いて、介入後の各年度と SSI 発生との関連を分析した。

結果

病院 319 施設における計 157,343 件の手術について分析した。JANIS プログラムに参加後、肝切除術および心臓手術などの手技において標準化感染比は低下した。JANIS プログラムへの参加は、特に 3 年後の複数の手技で標準化感染比の低下と有意に関連した。介入後 3 年目のオッズ比(基準:介入前 1 年)は、結腸手術では 0.86(95%信頼区間[CI]0.79 ~ 0.84)、幽門側胃切除術では 0.72(95%CI 0.56 ~ 0.92)、胃全摘術では 0.77(95%CI 0.59 ~ 0.99)であった。

結論

JANIS プログラムへの参加により、3 年後に日本の病院の複数の手技における SSI 予防実績の向上が認められた。

JANIS プログラムへの参加は、SSI 予防実績の向上を得るための各医療施設での改善プログラムの成果であろう。継続的に JANIS プログラムを運用することで PDCA サイクルを回し、さらなる改善を期待したい。
