Closed-incision negative-pressure wound therapy reduces superficial surgical site infections after open colon surgery: an NSQIP Colectomy Study

背景

閉鎖創における陰圧閉鎖療法(iNPWT)の使用は、この十年間で大腸切除術を含めた外科領域において増加している。

目的

大規模全国データベースを用いて、開腹大腸切除術後の iNPWT の使用に関連する術後転帰を比較すること。

方法

米国外科学会(ASC)National Surgical Quality Improvement Program(NSQIP)の Targeted Colectomy Database を用いて、2015 年から 2020 年の間に手術を受けた患者の後向きレビューを行った。術中における iNPWT の実施を、腹部開腹手術において切開創の皮膚を含む全層の閉鎖が行われた患者について同定した。傾向スコアマッチングを行って、iNPWT なしに切開創の全層の閉鎖が行われた対照患者を特定した。患者を 1:4(iNPWT 対 対照)の比でマッチさせ、また術後の表層、深部および臓器・体腔手術部位感染症(SSI)、創離開、および再入院について比較した。

結果

マッチさせたコホートとして 1,884 例を選択した。iNPWT を受けた患者は手術時間中央値が長かった(170[四分位範囲:129 ~ 232]対 161[四分位範囲:114 ~ 226)分、P < 0.05)。iNPWT を受けた患者では受けなかった患者と比較して、30 日間の表層手術創 SSI(3%対 7%、P < 0.05)および再入院(10%対 14%、P < 0.05)の発生率が低かった。iNPWT により、深部 SSI、臓器・体腔 SSI、または創離開のリスクは低下しなかった。

結論

手術時間はわずかに延長したが、開腹大腸切除術における iNPWT の使用は、表層 SSI および 30 日再入院率のオッズの低下と関連した。このことから、iNPWT は、患者アウトカムを改善するために開腹大腸手術においてルーチンで使用すべきであることが示唆される。
