手術室環境において滅菌された物品をカバーすると空気中の細菌による汚染が減少する効果がある:システマティックレビューおよびメタアナリシス★★

背景
術後の手術部位感染症は深刻な問題である。手術を待つ間、滅菌された物品をカバーすることが、空気中の細菌による汚染から物品を保護するために重要である可能性がある。
目的
5つのデータベースを対象に、手術室および手術器具の細菌汚染に関連する検索語を用いたシステマティックレビューを実施し、この方法の有効性を評価した。
方法
MEDLINE、Cochrane、CINAHL、Embase、Web of Science のデータベースにおいて、開始から 2023 年 2月 13 日までの、手術室環境で実施されたカバー介入に関するランダム化および非ランダム化対照研究を検索した。評価項目は、コロニー形成単位として測定された空気中の細菌による汚染とし、異なる対象期間でメタアナリシスを実施した。本システマティックレビューおよびメタアナリシスは PRISMA 声明に従って報告し、プロトコールはあらかじめ PROSPERO に登録した(CRD42022323113)。時点は 30 分から 24 時間の範囲であった。
結果
結果から、滅菌された物品をカバーすることで、カバーしない場合と比較して、空気中の細菌による汚染が有意に防止されることが示された。メタアナリシスでは、空気中の細菌による汚染から保護するために、滅菌された物品をカバーすることが支持され、効果の大きさの Z は 4.76 であった(P < 0.00001、信頼区間 -1.94 ~ -0.81)。不均一性解析において、不均一性は 83%であった。
結論
細菌汚染に関してマイナスの影響は認められなかったことから、滅菌カバーを用いた保護により、手術の開始を待つ間の、滅菌された物品の空気中の細菌による汚染が減少すると結論付けられる。
監訳者コメント:
手術室環境がいかなる細菌環境になっているのか、今一度良く理解する必要がある。日本手術医学会から出ている「手術医療の実践ガイドライン(改訂第三版)」の第 9 章 手術室環境整備にも、『一般的には空中に浮遊する細菌が術野に落下して感染を起こす危険性は必ずしも高くはない。しかし、手術器材の汚染や環境的要因が多少とも関与している術後感染を防止するために、手術室環境清浄化は患者に対する感染の危険性を軽減するうえで重要である。』と明記されているように手術室環境の衛生状態を熟知する必要がある。
