集中治療室における汚染された排水管に関連したカルバペネマーゼ産生菌の長期にわたるアウトブレイク

背景
2018 年から 2022 年にベルギーの 3 次病院で、成人集中治療室（ICU）の入院患者における、VIM 産生緑膿菌（Pseudomonas aeruginosa）（PA-VIM）および NDM 産生腸内細菌目細菌（CPE-NDM）を主とする、カルバペネマーゼ産生菌の感染が大きな問題になりつつあった。
目的
複数の菌種および持続的な環境リザーバが関与する、この ICU における長期間のカルバペネマーゼ産生菌アウトブレイクを調査した。
方法
積極的症例探索、環境試料採取、患者株および環境株の全ゲノムシークエンシング（WGS）解析、実行した制御戦略について、本研究で説明した。
結果
2018 年から 2022 年に、患者 37 例が ICU 入室中に PA-VIM および／または CPE-NDMの保菌者または感染者となった。WGS によって、排水管から採取された臨床株と環境株の疫学的関連が確認され、クローン株の伝播と、プラスミド結合および／またはトランスポゾンジャンプを介する遺伝子の水平伝播が認められた。四級アンモニウム系消毒剤による環境消毒および汚染された装備の交換では、環境感染源は根絶されなかった。興味深いことに、四級アンモニウム化合物に対する耐性を付与する排出ポンプ遺伝子が、分離株に幅広く認められた。シンクを除去することは不可能であったため、バイオフィルムを分解する発泡剤と、過酢酸および過酸化水素ベースの発泡消毒剤の併用を評価し、これまでのところ、隣接する排水管へのカルバペネマーゼ産生菌の再定着は防止されている。
結論
可動遺伝因子を伝播する能力をもつ抗菌薬および消毒薬耐性菌の病院環境における残留は、ICU 患者にとって重大な脅威となり、流行状態に移行するリスクがある。持続的な環境リザーバに対処し、カルバペネマーゼ産生菌の伝播を防止するためには、革新的戦略が必要である。
