Novel scores relevant to antimicrobial resistance and hospital-acquired infections developed with data from a multi-hospital consortium in the Parisian region of France

目的

抗菌薬耐性および医療関連感染症における差をベンチマーキングして比較および理解するための指標は、病院にとっての優先事項を明らかにする上で極めて重要である。

方法

本研究は、フランスのパリ地域にある大規模な公立病院コンソーシアムにおいて、病院獲得感染症または抗菌薬耐性グラム陰性桿菌による血流感染症（GNB-BSI）の発生率を測定した。

結果

各病院内において、抗菌薬耐性 GNB による GNB-BSI の発生率と、病院獲得 GNB-BSI の発生率との間に強い正の相関が認められた。抗菌薬耐性の発現率と医療関連感染症の発生率を測定する 2 つのスコアが、異なる GNB-BSI 発生率を組み合わせることで、指標として開発された。両スコアには、この病院コンソーシアム内で大きなばらつきがあった。多変量解析では、抗菌薬耐性スコアと医療関連感染症スコアは、手術用ベッドの割合、スタッフの欠勤、および擦式アルコール製剤の消費量と有意に関連していた。また、後者 2 つは、介入で修正可能な 2 つの特性であった。カルバペネム系薬の使用も抗菌薬耐性と関連したが、その理由は、カルバペネム系薬が耐性菌感染症の治療に好んで用いられる薬剤であることによる可能性がある。

結論

これらの結果は、病院研究における抗菌薬耐性の発現率および医療関連感染症の発生率に光を当てるものであり、医療施設ごとに対象を絞った介入を行える可能性を示唆している。

フランスのパリ地域にある大規模な公立病院コンソーシアムにおいて、病院獲得感染症または抗菌薬耐性グラム陰性桿菌による血流感染症（GNB-BSI）の発生率を測定した研究。各病院内において、抗菌薬耐性 GNB による GNB-BSI の発生率と、病院獲得 GNB-BSI の発生率との間に強い正の相関が認められた。結果は 2019 年に限定されたデータから得られているので、COVID-19 の影響を受けていない。（結果は示されていないが、COVID-19 流行中である 2020 年の AMR スコアに関連する変数に対しても同様の結果だったとのこと。）
