Virucidal activity of oral, hand, and surface disinfectants against respiratory syncytial virus

背景

RS ウイルスは、成人および小児における呼吸器感染症の主な原因として知られている。ヒト‐ヒト間伝播は、飛沫や直接的・間接的接触（汚染表面または医療従事者の手指など）を介して起こる。したがって、適切な衛生対策およびウイルス不活化についての知識がきわめて重要である。

目的

RS ウイルスの殺菌プロファイルを解明すること。

方法

本研究では、RS ウイルスに対して、とくに小児用にデザインされた含嗽液、世界保健機関（WHO）推奨の手指擦式製剤、エタノールおよび 2-プロパノールの殺ウイルス効果を定量的懸濁試験（EN14476）に基づき評価した。ステンレススチール製ディスク上の RS ウイルスの安定性を評価し、各種表面消毒薬（EN16777）によりその不活化を試験した。

結果

試験したすべての含嗽液は、1 種類を除き、ウイルス感染力価を定量下限まで低減させた。WHO 推奨の 2 種類の手指擦式製剤、および 30％エタノール、2-プロパノールは、感染ウイルスの検出を完全に消失させた。数日後、ステンレススチール製ディスク上の感染性 RS ウイルスが回収されたが、アルコール、アルデヒド、または過酸化水素ベースの表面消毒薬を用いたすべての試験で、RS ウイルスは効果的に不活化された。

結論

含嗽液、試験したすべての手指擦式製剤、表面不活化試薬は、in vitro で RS ウイルスの不活化に十分であった。

監訳者コメント：

RSV は飛沫のみならず接触（直接および間接）により拡大することがわかっており、手指衛生と高頻度接触面の消毒は重要視されている。エンベロープ（＋）の RNA ウイルスであるため、エタノールおよび 2-プロパノールについての消毒効果が認められた。同時にいわゆるマウスウォッシュの効果をみているが、これについては試験管内でのデータであり、実際にどれだけの効果（自己の感染予防効果と相手への感染予防効果の両者）があるのかは不明である。他方、RSV ワクチンが日本で2023 年 9 月に 60 歳以上で保険未収載ではあるが承認されており、消毒剤のみではなく、ワクチンでの予防策も選択肢に入った。
