Examining outpatients’ hand hygiene behaviour and its relation to COVID-19 infection prevention measures

背景

外来診療の需要が高まるにつれて、これらの施設において感染伝播のリスクが高まっている。しかし、外来診療所での感染予防制御策に関する調査には限界があり、患者に焦点を置いた調査はない。

目的

COVID-19 パンデミック時の外来患者の手指衛生行動、その決定因子、他の感染予防策とそれらの関連を調査すること。

方法

外来診療所 1 施設における 1 つの患者コホートの手指衛生行動を観察し、診療所 5 施設における各コホートの外来での手指衛生実践について調査した。理論的領域フレームワーク（TDF）に基づく質問票を用いて、行動の予測因子を調べた。さらに、患者は COVID-19 感染予防策の遵守、ワクチン接種状況、疾患リスクの認識、ワクチンへの躊躇について回答した。

結果

患者 618 例において観察された手指衛生率は低かったが（12.8％）、調査した患者 300 例のうち 67.3％は外来診療所に入る際に手指を消毒していると回答した。TDF 領域の「記憶、注意、意思決定プロセス」「感情」は、外来診療所における現在（今日の）および通常の手指衛生行動を有意に予測した。手指衛生行動は COVID-19 感染予防遵守と正の相関を示したが、患者のワクチン接種状況との有意な関連は認められず、行動の動機付けが異なることが示唆された。

結論

患者が忘れずに手指を清浄にすることに焦点を置いた介入により、外来患者における手指衛生は改善されるはずである。患者の安全を確実にするために、外来診療所での感染予防に関するさらなる調査が必要である。

監訳者コメント：

COVID-19のパンデミックにより世界の人々の衛生行動が変化した。ここドイツにおいては COVID-19 の診療は外来中心で実施されたが、一般診療所では感染予防策に対する十分なインフラやリソースがないのが現状である。結果的に外来に来る患者の手指衛生、マスク、環境消毒などの不十分さが外来での感染リスクを高めている。外来患者の手指衛生に関する研究はこれまでなかった。結果は手指衛生への意識は 7 割弱あるものの、実際には約 13％程度の実施率に留まっており、この差を縮めてゆくための戦略を考えてゆくことが必要であろう。
