カルバペネマーゼ産生グラム陰性菌の保菌者の追跡戦略のばらつき:国際的なスコーピング調査の結果★★

背景
感染予防・制御策を安全に中止するには、多剤耐性菌を保菌しなくなったことを確認する追跡戦略が必要である。カルバペネマーゼ産生グラム陰性菌の保菌者をいつ保菌していないと考えることができるかを決定するエビデンスに基づく基準は、現時点では存在しない。
目的
この研究では、カルバペネマーゼ産生グラム陰性菌の保菌者と分かっている者に対して現在使われている追跡戦略を評価した。
方法
2023 年 10 月から 2024 年2 月の期間にEuropean Society of Clinical Microbiology and Infectious Diseases(ESCMID)Study Group for Nosocomial Infections(ESGNI)を通じてカルバペネマーゼ産生腸内細菌目細菌(CPE)、カルバペネム耐性アシネトバクター・バウマニ(Acinetobacter baumannii)(CPAB)、カルバペネム耐性緑膿菌(Pseudomonas aeruginosa)(CPPA)に対する追跡戦略を開始した。調査項目は、追跡戦略と、スクリーニングを行わない基準および保菌解除の基準に関する 49 の質問とした。受動的な追跡は、機会があれば行うスクリーニングと定義し、一方、積極的な追跡は、前向きに計画された。また記述的分析を実施した。
結果
欧州 21 カ国と非欧州 6 カ国の 134 名の回答者が調査を完了した。CPE については、80%を超える施設が何らかの種類の追跡を実施していたがCPABとCPPAはそれぞれ66.4%、60.4%であった。受動的な追跡が最も頻度が高かった。受動的な追跡の大部分は、保菌者が病院に新しく入院してきたときに実施された。積極的な追跡は、84 ~ 92%で、最初の検出から 4 か月以内に開始された。スクリーニングで連続して陰性となることおよび最後の検出からの期間の両方が、保菌解除基準として最も多く報告された。しかし、20 ~ 35%では、保菌解除基準自体が存在しなかった。
結論
カルバペネマーゼ産生グラム陰性菌の保菌者の追跡についてコンセンサスは存在しない。予防方策をいつ中止するかを決定するには、エビデンスに基づいた戦略、あるいはコンセンサスが差し迫って必要である。
監訳者コメント:
日本でも同様に CPE 等のフォローアップ戦略は施設によって大きく異なる。本研究が示すように、エビデンスに基づいた統一基準の確立が急務である。特に、日本では高齢化社会により長期療養型施設との連携が重要であり、施設間での保菌者情報共有と標準化されたフォローアップ手順の確立が必要である。
