Tailored positioning and number of hand rub dispensers: the fundamentals for optimized hand hygiene compliance

背景

擦式アルコール製剤ディスペンサーが医療従事者の仕事の流れに適した位置で利用可能かどうかは、指示に基づいた擦式法を行うための決め手となる。擦式アルコール製剤ディスペンサーの配置に関する要件とガイドラインは整っているが、科学的なエビデンスは不足していることが多い。

方法

擦式アルコール製剤ディスペンサーの配置と数が手指衛生実行に及ぼす影響を分析することを目的として、ドイツのマールブルク大学病院の 38 床の外科病棟 1 棟の病室に、現在のディスペンサーの配置を補完するため、追加のディスペンサーを規則的に配置した。擦式アルコール製剤の使用は、NosoEx 手指衛生モニタリングシステムを用いて、補完の前後で継続的にモニタリングされた。介入前は病棟に 53 個のディスペンサーがあり、介入後は 82 個のディスペンサーがあった。

結果

ディスペンサーの追加によって、病棟全体の擦式アルコール製剤の使用量は増加した。介入前の平均使用量は 20.6 mL/患者日であったのに対し、介入後の平均使用量は 25.3 mL/患者日であった。ディスペンサーの配置の組み合わせに応じて、病室における的を絞った補充を介して使用量は増加した。1 病室あたり 2 個または 3 個のディスペンサーが存在すると、1 病室あたり 1 個のディスペンサーがある場合と比較して擦式アルコール製剤の使用量は有意に多くなった。望ましい配置の組み合わせは、入口 – ベッド前方 – ベッド後方および入口 – 足側の端であった。

結論

ディスペンサーの配置と数を最適化することによって、擦式アルコール製剤の使用量は有意に増加する可能性がある。はっきりした要因は視認性とディスペンサーの使用を仕事の流れに組み込めることであり、特にディスペンサーは入口部分に配置すべきである。病室で必要とされるディスペンサーの数と配置に関して、推奨を最適化すべきである。
