Evaluating the risk of Clostridioides difficile infection from toilet flushing: a quantitative microbial risk assessment and implications for infection control

背景

厳しい感染制御策にもかかわらず、クロストリジオイデス・ディフィシル(Clostridioides difficile)感染症(CDI)は、依然として、医療現場における難問の 1 つであり、トイレットプルーム・バイオエアロゾルのような伝播の媒介物が見過ごされていることがその一因となっている。

目的

水洗トイレを流すことによる CDI の伝播に関連するリスクを系統立てて定量化し、理解を深める重要な情報を CDI の予防対策に提供すること。

方法

制御されたトイレの環境において、インパクションによる検体採取を行ない、水洗後の空中 C. difficile を定量し、接触の多い表面のスワブを採取し、汚染のレベルを評価した。次に、定量的微生物リスク評価の手法を使い、定着していた人による汚染によってその次の使用者が曝されるリスクを推定した。

結果

1 回流すことで、C. difficile が空中に放出される可能性があり、バイオエアロゾルの濃度が 29.50 ± 10.52 cfu/m3 にまで達し、すぐ近くの表面に約 8 ~ 11 cfuが落下した。便器内の細菌濃度は、水洗後に 4.4 log 減少したにもかかわらず、便器の内壁には細菌が残っていた。相対湿度が、水洗後最初の 10 分以内に約 31.28 %増加したが、これは、エアロゾル化した C. difficile の生存能力と伝播を増強する可能性がある。病院の頻繁に使用する環境において、水洗のボタンに触れることと、吸入しその後に飲み込んでしまうことが、最もリスクが高く、US EPA や WHO による許容可能な感染リスクの閾値を超えている。

結論

この研究結果は、医療現場で見過ごされていたトイレットプルーム・バイオエアロゾルを介する CDI の伝播のリスクを軽減するには、現在のトイレのデザイン、公衆衛生政策、施設管理のやり方を見直す必要があることを示している。それに加えて、この研究は、感染制御のやり方において、行動や基盤となる設備にかなりの変化をもたらすための介入を、エビデンスに基づいて作り出す基礎となる。
