同時に存在する複数の感染経路に影響する新生児集中治療室におけるアデノウイルス D8 のアウトブレイク★★

背景
新生児集中治療室(NICU)におけるアデノウイルスのアウトブレイクは、持続的な伝播や重篤な有害転帰をもたらす可能性がある。本研究は、未熟児網膜症のスクリーニングにおいて使用される眼科機器と関連した、NICU でのアデノウイルス D8 型アウトブレイクの調査および制御について報告する。新生児、親、看護師において症例を観察した。
方法
アデノウイルスの DNA 分子の検出のために、患者、親、医療従事者のほかに環境のサンプリングも含め、アウトブレイクの調査を実施した。一部の親が一時的に滞在していた宿泊室も調査した。未熟児網膜症の検査と関連するリスクを評価するために、流行時期に入院した新生児に焦点を絞り、後向きコホート研究を実施した。
結果
新生児において 15 例の症例が特定され、1 例を除く全例が結膜炎を呈した。医療従事者 2 名、親 18 人が結膜炎に感染した。RetCam(訳者注:眼底撮影装置)の上部と、親が共有した冷凍庫で、アデノウイルス DNA が同定された。すべてのアデノウイルス陽性サンプルが、アデノウイルス D8 型に分類された。アデノウイルス感染症の発生率は、未熟児網膜症の検査を受けた新生児の方が高かった(37.8%[37 例中 14例]対 0.9%[110 例中 1 例]、P < 0.001)(相対リスク 41.6[95%信頼区間 5.7 ~ 305.8])。RetCam は、アデノウイルスに対して殺ウイルス性のある消毒薬 30 分接触させた後、次の検査を実施した。
結論
このアウトブレイクは、RetCam による未熟児網膜症の検査と有意に関連しており、この検査では、患児の入れ替わりの速さに消毒プロトコールがうまく適用されなかった。さらに、親から新生児への院内伝播や親間伝播が、症例の拡散の一因になっていた可能性もある。新たな消毒法が実施されてから、さらなる症例は認められていない。
監訳者コメント:
アデノウイルス性結膜炎の院内集団発生であるが、網膜を観察するための RetCam 眼底撮影装置は角膜に直接接触する部分が存在しセミクリティカル機器である。環境拭い検査にて装置のキーボードとハンドルからアデノウイルスが検出されており、汚染された装置を介して新生児が感染し、感染した新生児との接触感染により親にも拡がり、冷蔵庫を介した親同士の感染も考えられ、複数の感染経路がこのアウトブレイクにおいては関与している。使用後の 30 分の消毒時間が守られなかった結果であり、その後このアウトブレイクに対して感染制御チームは新たに二酸化塩素浸漬ワイプによる 30 秒間の拭き取り消毒とその後の滅菌水によるリンスを推奨した。これらの予防策実施後新たな発生はなくなった。
