集中治療室におけるポイント・オブ・ケア法としての T2 核磁気共鳴アッセイの精度★

背景
菌血症患者における病原体の同定を促進するため、新しい分子診断法が検討されている。
目的
集中治療室におけるポイント・オブ・ケア検査としての T2 核磁気共鳴(T2MR)アッセイ(T2 細菌[T2B]および T2 耐性[T2R])の実現可能性および診断精度を、血液培養ベースの検査と比較して評価すること。
方法
菌血症の疑いのある連続症例に関する前向き横断研究。参照方法として血液培養を用いて診断精度を評価した。
結果
計 208 例を本研究の対象とした。T2MR アッセイは血液培養ベースの方法と比較して、サンプル採取から報告までの平均時間が短かった(P < 0.001)。無効な報告の割合は、T2B アッセイでは 6.73%、T2R アッセイでは 9.9%であった。T2B アッセイでは、全体の陽性一致率(PPA)は 84.6%(95%信頼区間[CI]71.9 ~ 93.1%)、陰性一致率(NPA)は 64.3%(95%CI 55.4 ~ 72.6%)、陽性予測値(PPV)は 48.9%(95%CI 42.5 ~ 55.3%)、陰性予測値(NPV)は 91.2%(95%CI 84.4 ~ 95.2%)であった。Cohen のκ係数は 0.402 であった。T2R アッセイでは、全体の PPA は 80%(95%CI 51.9 ~ 95.7%)、NPA は 69.2%(95%CI 54.9 ~ 81.3%)、PPV は 42.9%(95%CI 31.7 ~ 54.8%)、NPV は 92.3%(95%CI 81.1 ~ 97.1%)であった。Cohen のκ係数は 0.376 であった。
結論
T2MR アッセイは菌血症の迅速な除外に関して NPV が高く、集中治療室におけるポイント・オブ・ケア診断検査として適用した場合、潜在的に抗菌薬適正使用支援に役立つ可能性がある。
監訳者コメント:
T2 核磁気共鳴アッセイは、培養に依存しない菌血症の分子診断法として期待される。
