Use of thermal imaging to measure the quality of hand hygiene

目的

手指衛生は、感染症伝播の最も効果的な予防法として以前から推奨されている。しかし、これまでの研究では手指衛生について遵守率が低く、その質が低いと報告されているため、医療従事者における手指衛生の遵守およびその質の継続的なモニタリングが極めて重要である。本研究では、サーマルカメラを RGB カメラと併用することで、アルコール擦式製剤の手指への塗布状況を検出し、これにより擦式手指消毒の質をモニタリングすることが実施可能であるかを検討した。

方法

参加者計 32 名が、本研究への参加に登録した。参加者にはアルコール擦式製剤の様々な手指への塗布状況を検討できるように 4 種類の擦式手指消毒の課題を実行するよう指示した。各課題の後、参加者の手指をサーマルカメラおよび RGB カメラで撮影し、紫外線検査を用いてアルコール擦式製剤の手指分布について実測データを得た。深層学習ニューラルネットワークである U-Net を用いて、熱画像からアルコール擦式製剤に曝露された領域をセグメント分けし、熱画像と紫外線画像における分布の違いについて正確度と Dice 係数を用いて比較し、システムの性能を評価した。

結果

本システムには、擦式手指消毒の実施から 10 秒後に観察した場合の正確度（93.5％）および Dice 係数（87.1％）について有望な結果が認められた。擦式手指消毒から 60 秒後の観察では、正確度は 92.4％、Dice 係数は 85.7％であった。

結論

熱画像の使用には、手指衛生の質について正確かつ一貫した系統的なモニタリングを実施できる可能性がある。

監訳者コメント：

手指衛生の評価には、適切な場面での実施状況と消毒剤の手指全体への塗布状況の二つの評価方法があり、どちらも感染予防の観点からは重要である。本研究は後者で、速乾性アルコール手指消毒剤の擦り込み後のアルコールの蒸発時の皮膚温低下を熱画像で捉え、手指消毒の質評価を試みたもので、これまでのブラックライトと蛍光塗料による煩わしさはなくなる。しかしながら、本研究は実験室ベースであり、医療現場というリアルワールドでのトライアルはこれからであり、結果が期待される。
