背景

新型コロナウイルス感染症(COVID-19)パンデミックは、これまで病院の感染制御実践に影響を及ぼしてきた。

目的

COVID-19 パンデミックが集中治療室(ICU)における医療関連感染症(HAI)に及ぼす影響について評価すること。

方法

韓国国家医療関連感染サーベイランスシステム(KONIS)のデータを用いて後向き解析を行った。血流感染症(BSI)、中心ライン関連血流感染症(CLABSI)、カテーテル関連尿路感染症(CAUTI)、および人工呼吸器関連肺炎(VAP)の発生率および微生物分布について COVID-19 パンデミック前とパンデミック中の比較を、病院規模に応じて実施した。

結果

BSI の発生率は、COVID-19 パンデミック中には COVID-19 パンデミック前と比べて有意に減少した(10,000 患者日当たり 1.38 対 1.23、相対的変化量-11.5%、P < 0.001)。VAPの発生率は、COVID-19 パンデミック中には COVID-19 パンデミック前と比べて有意に減少した(1,000 デバイス日当たり 1.03 対 0.81、相対的変化量-21.4%、P < 0.001)一方、2 つの期間の間で CLABSI(1,000 デバイス日当たり 2.30 対 2.23、P = 0.19)および CAUTI(1,000 デバイス日当たり 1.26 対 1.26、P = 0.99)の発生率は同等であった。BSI および CLABSI の発生率は、大規模病院では COVID-19 パンデミック中には COVID-19 パンデミック前と比べて有意に増加した一方、両者の発生率は、小規模から中規模の病院では有意に減少した。CAUTI および VAP の発生率は、小規模病院でのみ有意に減少した。2 つの期間の間で、医療関連感染症患者から分離された多剤耐性病原体の検出率には、有意な変化傾向はなかった。

結論

集中治療室における BSI および VAP の発生率は、COVID-19 パンデミック中には COVID-19 パンデミック前と比べて減少した。この減少は主として、小規模から中規模の病院で認められた。

COVID-19 パンデミックの医療関連感染症(HAI)発生率への影響の報告が各国からなされている。医療負荷増大に伴う HAI 発生率の増加や逆にパンデミック中に強化された感染対策の影響によると考えられる HAI 発生率の減少などのさまざまな報告がある。本研究では韓国のナショナルデータによる ICU での HAI の発生率を病院規模別に見ているが、パンデミックの影響は軽微であると考察されている。
