手指の有機物負荷を減少させ製品を乾燥した手指に適用可能にすることで臨床の現実に近づけた擦式アルコール製剤のための欧州規格 EN 1500 試験法の発展★

背景
手指衛生製品の殺菌効果を評価するために現在欧州や北米で使用されている方法には制限があり(例えば試験菌の選択、汚染方法)、実際の臨床効果を予測できる方法はない。そのため、世界保健機関はよくある臨床の現実をより正確に反映する方法の開発を提唱している。
方法
実験 1 では 60% v/v イソプロパノールを用いて、2 種類の汚染方法(欧州規格 EN 1500 による液浸法、米国規格 ASTM E2755 による低容積法)を欧州規格 EN 1500 の試験菌である大腸菌(Escherichia coli)で検討した。実験 2 では、エンテロコッカス・フェカーリス(Enterococcus faecalis)を用いて 2 種類の汚染方法を比較した。実験 3 では、低容積汚染法を用いて 2 種類の試験菌を比較した。対応のあるサンプルの Wilcoxon 検定を用いて各実験のデータを比較し、全実験のデータを統合して線形混合効果モデルにフィットさせた。
結果
混合効果分析によって、試験菌と汚染方法の両方が前値に影響し、3 つの因子すべてが log10 減少に影響を及ぼすことが確認された。前値が高いと有意に高い log10 減少値が得られ、液浸は有意に高い log10 減少値に寄与し、大腸菌は有意に低い log10 減少値を示した。
結論
低容積汚染法を用いた E. faecalis に対する有効性評価は、欧州規格 EN 1500 の代替として検討できる可能性がある。これによって、グラム陽性菌を含み汚染負荷を減少させることで試験法の臨床的適合性の向上が促進され、現実に近い製品の適用が可能となる可能性がある。
監訳者コメント:
EN 1500 と ASTM E2755 は、実際の臨床的な殺菌効果を予測できない。擦式アルコール製剤を乾燥した手指に適用すること、手指の有機物を減少させること、グラム陽性菌を追加することで、臨床現場に近づけた条件で評価しうる。
