Infection prevention and control between legal requirements and German Society for Hygiene and Microbiology expert assessments: a cross-sectional study in September-November 2022

背景

重症急性呼吸器症候群コロナウイルス-2（SARS-CoV-2）のパンデミックの初期と大いに異なり、現在、さらに多くの病院の問題が政策によって規制されている。

目的

感染予防・制御（IPC）戦略に関する専門家の推奨と法的要件の違いを明らかにすること。

方法

2022 年 9 月 29 日から 2022 年 11 月 3 日の間に、ドイツ衛生・微生物学学会の会員 1,319 名を対象に横断研究を実施した。回答率は 12％であった。本稿では異なる IPC 戦略に関する専門家の推奨について報告する。

結果

SARS-CoV-2 のパンデミック後でも、大部分の専門家（66％）は普遍的なマスクの使用を推奨し、34％は季節的なマスクの使用を推奨した。医療微生物学の専門家は、IPC の専門家と比較してマスクの着用を無期限に継続することを推奨する傾向が強かった。マスクの種類に関して、IPC の専門家（47.3％）は医療用マスクを推奨する頻度が高かったのに対し、医療微生物学の専門家（31.8％）は FFP2 マスクを好んだ。大部分の専門家（54.7％）は、主に極めて脆弱な患者のいる環境で、地域の発生率が高い場合に、週に 2 回の頻度で従事者の普遍的スクリーニングを行うことを推奨した。SARS-CoV-2 に曝露される従事者に対する優勢なアドバイス（専門家の 50％以上が推奨）は、曝露期間にかかわらず、毎日の検査とマスクの着用であった。

結論

専門家の推奨は法的要件から逸脱しており、より区別され一定の比率を持つように思われた。個別の経験と専門知識がマスクの推奨に及ぼす影響について、さらに詳細に調査すべきである。関連する政策決定に関して、専門知識による迅速で焦点を合わせた広範囲にわたる協議は付加価値となる可能性がある。

監訳者コメント：

2022 年 9 月末から 11 月初旬にかけてドイツ微生物学会会員により実施された調査であり、回答者の約 6 割がリーダー的役割で直接医療に携わっていない研究者であり、回収率が 12％と低いことなどから、結果を解釈するにあたってこれらの背景を考慮する必要がある。また、感染予防策においては、法的要件と専門家の意見とが異なっている部分もあり、感染対策の将来的な方向性を決める上で実践的な面に配慮が必要であり、それを明らかにした重要な論文である。
