血液透析患者における黄色ブドウ球菌（Staphylococcus aureus）保菌者に標的を絞ったムピロシンによる除菌とムピロシン耐性の続発リスク：20 年間にわたる縦断研究★★

血液透析を受けている黄色ブドウ球菌（Staphylococcus aureus）保菌者のムピロシンによる除菌は、ムピロシン耐性の懸念のため広く実施されてはいない。当院の血液透析室では、黄色ブドウ球菌の普遍的スクリーニングと標的を絞ったムピロシンによる除菌を組み合わせた戦略を 20 年前に導入した。血液透析患者を対象とする本研究では、2 つの期間中に黄色ブドウ球菌の血液および保菌分離株のムピロシン耐性を評価した。
黄色ブドウ球菌におけるムピロシン耐性は、血液分離株と保菌分離株の両方においてまれであった。さらに、2003 年から 2021 年の間に、黄色ブドウ球菌血流感染症の年間発生率の低下傾向が認められた。血液透析を受けている黄色ブドウ球菌保菌者に標的を絞ったムピロシンによる除菌は、医療関連感染を予防するための持続可能な対策の 1 つである。
監訳者コメント：
血液透析患者における黄色ブドウ球菌保菌は血流感染等の感染症リスクが高まる結果、予後に影響を及ぼし、除菌により 80％血流感染が減ることが報告されている。ムピロシン塗布での問題はムピロシン耐性の問題であるが、本論文では約 20 年間除菌療法を実施しているが、耐性出現率は極めて低く抑えられ、黄色ブドウ球菌による血流感染も低下している。なお、本論文でのムピロシン除菌治療は、（2％ムピロシン鼻腔塗布 2 回/日 ＋ 4％クロルヘキシジン・シャワー毎日） × 7 日間である。
