抗菌薬適正使用支援プログラムの対象となった新生児集団における抗菌薬使用の評価：システマティックレビュー★

背景
新生児の敗血症治療における無制限の抗菌薬使用は誤用の原因となり、こうした不適切な抗菌薬使用を減らす方策として処方プログラムの最適化が勧められる。
目的
新生児医療における抗菌薬の使用量について、適切な抗菌薬使用を目的とした介入プログラム（抗菌薬適正使用支援プログラム）の影響を評価すること。
方法
論文の検索を、電子データベースにおいて、また最初に特定された発表論文中の引用文献の手動検索により行った。検索した電子データベースは、BVS（Virtual Health Library）、Cochrane Library、Embase、MEDLINE/PubMed、SciELO、Scopus および Web of Science であった。論文の組み入れについて、日付や期間の制限は設けなかった。論点とする PICO は、新生児集中治療室に入室した新生児の患者集団（P）、抗菌薬療法を最適化するための介入プログラム（I）、同プログラムを受けなかった新生児との比較（C）、抗菌薬使用に関する転帰（O）と定義した。
結果
最初のデータベース検索の結果、論文 1,223 報が得られた。論文のスクリーニングを行い、本研究の主題に関連する原著論文 16 報を選択した。これらは、本研究の対象集団における抗菌薬使用について定量的アプローチを実施していた。ほとんどの論文が、抗菌薬使用の主要な指標として治療日数（DOT）を用いており、Newcastle-Ottawa Scale により質が高いと判定された。すべての研究が単一の地域病院で実施されており、そのほとんどが高所得国に所在していた。
結論
検索で特定されたすべての研究のうち、新生児医療における抗菌薬使用を評価したものはほとんどなかった。新たな研究が必要であり、DOT が抗菌薬使用を評価するための最も適切な指標であることが示された。
監訳者コメント：
新生児領域の抗菌薬適正使用支援プログラム、敗血症評価方針等は、抗菌薬の消費量を削減し、多剤耐性病原体に関連する感染症に起因する罹患率と死亡率を減らすことができ、患者の安全のために処方を最適化するために不可欠な要素となる。新生児領域での質の高い研究は限られており、今後さらなる研究が必要である。
