心臓手術時の胸骨創における皮膚細菌の存在とインプラント材料の汚染★

背景

胸骨創感染症および大動脈人工血管感染症は、心臓手術後の重篤な合併症である。黄色ブドウ球菌(Staphylococcus aureus)およびコアグラーゼ陰性ブドウ球菌は、胸骨創感染症の原因として最も多くみられるものである一方、大動脈人工血管感染症に関する研究はより少ない。大動脈人工血管感染症は、手術時または術後の血行性散布時の汚染から発生する可能性がある。皮膚共生細菌、例えばキューティバクテリウム・アクネス(Cutibacterium acnes)などが手術創内に存在するが、それが感染症を引き起こし得るかどうかについては議論がある。

目的

胸骨創における皮膚細菌の存在について検討すること、ならびに皮膚細菌が手術用の材料を汚染することがあるかについて評価すること。

方法

2020 年から 2021 年に Örebro University Hospital で冠動脈バイパス術および/または心臓弁置換術を受けた患者 50 例を組み入れた。手術中の 2 つの時点で皮膚および皮下組織から、また皮下組織に押し付けた人工血管片およびフェルト生地から培養を実施した。最も一般的な細菌分離株について抗菌薬感受性試験を、ディスク拡散法と勾配法により実施した。

結果

皮膚の培養では、手術開始時には患者の 48%、2時間後には患者の 78%で増殖が認められ、皮下組織の培養ではそれぞれ患者の 72%および 76%が陽性となった。最も多く認められた分離株は C. acnes および表皮ブドウ球菌(Staphylococcus epidermidis)であった。手術用材料の培養では、80 ~ 88%が陽性となった。S. epidermidis 分離株については、手術開始時と 2 時間後の比較で差は認められなかった。

結論

以上の結果から、皮膚細菌が胸骨創内に存在しており、心臓手術時に外科用グラフト材料を汚染している可能性が示唆される。

監訳者コメント


手術創は患者自身の皮膚細菌によって汚染されるだけでなく、手術中に人工血管等の手術材料も汚染される可能性がある。
