敗血症疑い患者における中心静脈カテーテル汚染率:観察臨床研究★★

背景
欧州では、毎年推定で中心ライン関連血流感染症(CLABSI)が 16 万件以上発生しており、死亡は約 2 万 5 千件に至っている。
目的
集中治療室の CLABSI 疑い例における輸液セットの汚染状況を明らかにすること。
方法
CLABSI 疑いのある集中治療室患者(2017 年 2 月から 2018 年 2 月)において、サンプル採取を行ったすべての中心静脈カテーテル(CVC)の 4 つのセグメント(CVC チップから接続した点滴回路)の汚染について調べた。二値ロジスティック回帰を用いて、危険因子分析を実施した。
結果
連続してサンプル採取した、部品 1,004 個を含む CVC 52 本を分析したところ、部品 45 個が少なくとも 1 つの微生物について陽性であった(4.48%)。カテーテル留置期間には、汚染リスクの 1 日当たり 11.5%の上昇(オッズ比 1.115)との有意な関連が認められた(P = 0.038、N = 50)。72 時間以内のCVC 操作回数は平均で 40 回(標準偏差 20.5)であり、汚染リスクとの関連はなかった(P = 0.381)。CVC のセグメントにおける汚染リスクは、近位部から遠位部になるに従って低くなった。CVC の交換不可能な部品には、高いリスクが認められた(14 倍、P = 0.01)。CVC チップの培養陽性と輸液セットにおける微生物増殖との間には、有意な正の相関が認められた(r(49) = 0.437、P = 0.001)。
結論
血液培養陽性となるのは CLABSI 疑い患者のごく少数であるが、CVC および輸液セットにおける汚染の発生率は高く、おそらくかなりの過少報告があると考えられる。隣接セグメントにおける同一微生物種の検出は、回路内での上流および下流への微生物の拡散の関与を反映している。したがって、無菌操作の重要性を強調すべきである。

本研究では、CLABSI が疑われる ICU 患者の CV カテーテルと点滴回路の部位別に微生物汚染を評価した。微生物汚染は回路内で拡がるので、CLABSI を疑う場合、CVC と点滴回路全体の交換を推奨している。
