Time to intestinal clearance of carbapenemase-producing Enterobacterales in hospital patients: a longitudinal retrospective observational cohort study

背景

カルバペネマーゼ産生腸内細菌目細菌(CPE)の腸内クリアランス(IC)は、病院において CPE 保菌患者に対する隔離予防策の中止を決定するための基礎となる。本研究は、自然な CPE-IC までの時間を評価し、それに関連する可能性のあるリスク因子を特定することを目的とした。

方法

この後向きコホート研究は、2018 年 1 月から 2020 年 9 月にかけて、3,200 床の 3 次教育病院において、CPE の腸内保菌が確認された全患者を対象に実施した。CPE-IC は、直腸スワブ培養で連続して少なくとも 3 回の CPE 陰性が認められ、その後に陽性結果がないことと定義した。CPE-IC までの時間中央値を決定するため、生存分析を実施した。CPE-IC に関連する因子を探索するため、多変量 Cox モデルを構築した。

結果

患者計 110 例が CPE 陽性で、うち 27 例(24.5%)が CPE-IC を達成した。CPE-IC までの時間中央値は 698 日であった。単変量解析から、女性(P = 0.046)、初回培養における複数の CPE 種(P = 0.005)、大腸菌(Escherichia coli)またはクレブシエラ(Klebsiella)属菌(それぞれ P = 0.001、P = 0.028)が、CPE-IC までの時間と有意に関連することが示された。多変量解析では、初回培養における大腸菌のカルバペネマーゼ産生、または CPE の ESBL 遺伝子保有が認められた場合に、CPE-IC までの時間中央値がそれぞれ延長することが明らかになった(補正ハザード比[aHR]0.13、95%信頼区間[CI]0.04 ~ 0.45、P = 0.001、および aHR 0.34、95%CI 0.12 ~ 0.90、P = 0.031)。

結論

CPE の腸内除菌は、数か月から数年かかることがある。カルバペネマーゼ産生大腸菌は、おそらく種間の遺伝子水平伝播を介して、腸内除菌を遅らせる主要な原因となると考えられる。したがって、CPE 保菌患者の隔離予防策の中止については慎重に検討すべきである。

監訳者コメント


フランスでの 3 次大学病院における CPE の腸内クリアランス(CPE-IC)の期間を調べた研究。先行報告では CPE-IC までの期間は 27.5 日から 295 日、CPE-IC の達成率も 8 から 61%であり、先行報告とは CPE-IC の定義が異なるので単純には比較できないが、本研究からは CPE-IC に数か月から数年かかることがわかる。また、カルバペネマーゼ産生、もしくは CPE の ESBL 遺伝子保有する大腸菌で、保菌が長期化することが示唆されている。筆者らが結論で述べているように、CPE 保菌患者の隔離予防策の中止については慎重に検討すべきである。
