高病原性肺炎桿菌（Klebsiella pneumoniae）感染症の予測因子：システマティックレビューおよびメタアナリシス★★

高病原性肺炎桿菌（hvKp）感染症は、顕著な病的状態および死亡をもたらす。感染症が hvKp 株または典型的な肺炎桿菌（Klebsiella pneumoniae）株のいずれに起因するかを明らかにする鑑別診断は、適切な臨床診療および感染制御策を実施する上で特に重要である。
目的
hvKp 感染症の潜在的な予測因子を特定し、評価すること。
方法
2000 年 1 月から 2022 年 3 月までの関連するすべての公表文献を、PubMed、Web of Science、Cochrane Library のデータベースを用いて検索した。検索用語は、次の用語の組み合わせを用いた：（i）Klebsiella pneumoniae または K. pneumoniae（ii）hypervirulent または hypervirulence。3 報以上の研究でリスク比が報告されている因子のメタアナリシスを実施し、1 つ以上の統計学的に有意な関連を特定した。
結果
観察研究 11 報を対象とした本システマティックレビューにおいて、肺炎桿菌感染症の患者計 1,392 例と、そのうち hvKp 株を有する患者 596 例（42.8％）を評価した。メタアナリシスでは、hvKp 感染症の予測因子は糖尿病と肝膿瘍であった（それぞれの統合リスク比：2.61［95％信頼区間 1.79 ～ 3.80］、9.04［2.58 ～ 31.72］、すべてP ＜ 0.001）。
結論
前述の予測因子の病歴を有する患者では、hvKp 保有の可能性を考慮して、複数の感染部位および／または転移の広がりの検査、早期の適切な原因制御策の実施など慎重な管理を開始すべきである。今回の調査は、hvKp 感染症の管理について臨床的認識の向上が急務であることを強調するものと確信している。
監訳者コメント：
肺炎桿菌感染症を、hvKp 感染症とそれ以外とを鑑別することは、患者の治療ならびに予後に関して大変重要である。糖尿病と肝膿瘍の予測因子におけるhvKp 株について、覚えておきたい。
