内科集中治療室における、他の患者やシンク配水管に由来する緑膿菌(Pseudomonas aeruginosa)に汚染された患者による稀な伝播イベントが、ゲノムベースのタイピングから明らかに★★

背景
ゲノムベースのタイピングデータを用いて、流行状態でない状況で、内科集中治療室に長期入院している患者による緑膿菌(Pseudomonas aeruginosa)獲得の経路を明らかにすることを目的とした。
方法
本単一施設前向き研究は、2019 年の 10 か月にわたり、標準的な衛生予防策を適用している 15 床の内科集中治療室で実施した。すべての水使用箇所に設置されたロック可能なシンク排水管を、週 2 回、漂白剤で消毒した。すべてのシンク配水管から週 1 回のサンプリングを行い、緑膿菌の環境分離株 404 個を収集し、保菌または感染患者(N = 65)のすべての緑膿菌分離株(N = 115)を収集した。すべての分離株について表現型における耐性プロファイルを明らかにし、ゲノムのシークエンシングを行った上で、その結果に基づいて耐性の決定因子を特定し、また収集した分離株の集団構造についてヌクレチドレベルで評価して、緑膿菌伝播イベントを特定した。
結果
すべてのシンク配水管が緑膿菌陽性で、各シンク配水管には数か月にわたり 1 つ以上のクローンの定着が認められた。ゲノムデータおよび時間空間データを組み合わせたところ、伝播の可能性があるイベントとして、シンク配水管から患者への伝播 1 件(65 件中 1 件、1.5%)、ならびに患者間の交差感染から患者 5 例の汚染に至った伝播 6 件(65 件中 5 件、7.7%)が同定された。伝播されたすべての分離株に、βラクタム系薬およびアミノグリコシド系薬に対する高い感受性が認められた。
結論
ゲノムベースのタイピングから、内科集中治療室においてシンク配水管に対する漂白剤による方策を用いた場合、シンク配水管による患者の緑膿菌汚染は稀(1.5%)であること、また他の患者に由来する緑膿菌を獲得していたのは患者の 7.7%のみであることが明らかになった。
監訳者コメント:
ICU における緑膿菌汚染は日常的に発生しており、全ゲノム解析の結果に、検出時期とベッドの位置と患者とのデータを組み合わせることで、環境から環境、ヒトから環境あるいはその反対、ヒト・ヒト感染など、感染経路が明確となるとともに、抗菌薬耐性遺伝子についても同時に検討可能である。次世代シーケンサーによる全ゲノム解析は、これまでの PFGE や MLST による解析よりもより正確な判定が可能となり、クロナリティの偽陽性や偽陰性を最小限にすることができる。そして、適切な感染対策実施においても重要な役割を果たしている。しかしながら、費用面において安くなってきてはいるものの、依然として高価であり、院内で発生するアウトブレイク調査に簡単には使えない状況があるのも事実である。
