背景

院内 SARS-CoV-2 感染症がもたらす影響は 2020 年以降に大きく変化した。しかし、感染制御の方針にとって適切な指針とするために必要な、院内 SARS-CoV-2 感染症の疫学に関する最新のエビデンスが不足している。

目的

SARS-CoV-2 感染症の二次感染率および様々な懸念される変異株における関連死亡率を明らかにすること。

方法

2020 年 12月 31 日 から 2021 年12 月 31 日にかけて、すべての院内 SARS-CoV-2 曝露イベントに関する単一施設後向き研究を実施した。SARS-CoV-2 感染症の院内獲得についての二次感染率および陽性までの時間を算出した。陽性接触者について 全原因による30日死亡率を評価した。

結果

連続したインデックス曝露イベント計 346 件を調べ、感染可能性のある接触者 1,378 例を特定した。感染可能性のある接触者 200 例が SARS-CoV-2 感染症を発症した(二次感染率 15.5%)。インデックス症例の大半(59%)は、二次感染による SARS-CoV-2 感染症を発症させなかった。濃厚接触者が SARS-CoV-2 感染症を発症した場合、80%がインデックス症例との最終接触後 5 日以内に検出された。陽性接触者における2021 年内の全関連死亡率は 9%で、オミクロン株伝播が多い時期をアルファ株伝播が多い時期と比較したところ、推定で 68%減少したことが認められた。

結論

本研究の結果は、ほとんどの SARS-CoV-2 感染症はインデックス症例との接触後 5 日以内に検出されること、またオミクロン変異株の場合は以前の変異株と比べて死亡率が大幅に低いことを示した。これらの結果は、感染制御プロトコールのための情報を提供して支援することで、病院全体での活動を可能にし、患者と接触するケアを安全なものにするうえで、重要な意義を有している。

監訳者コメント:

英国の単一施設での 2021 年の院内 COVID-19 症例の伝播動態および死亡率の報告。英国では 2021 年初頭はアルファ株、5 月から 11 月にかけてはデルタ株、年末にかけてはオミクロン株が優位であり、ワクチン接種も進めらている最中で、現在の状況とは異なるが、今度、オミクロン株流行下で多くが既感染者となった状況との比較で参考となる報告である。
