分子診断により標的とすることが多い遊離核酸に対する市販の消毒薬の分解能の評価★

背景
ポリメラーゼ連鎖反応（PCR）は病原体の迅速な検出のために不可欠な技術であるが、残存核酸による交差汚染の影響を受けやすく、偽陽性の結果につながる。これを防止するには、表面の十分な汚染除去が有用であるが、大部分のプロトコールは、遊離核酸よりも感染性微生物を標的としている。本研究の目的は、代表的な各種核酸に対する市販の表面消毒薬の分解能を評価することである。
方法
さまざまな有効成分を含む市販の表面消毒薬および 10％塩素系漂白剤を試験した。ステンレス鋼のクーポン上で核酸を乾燥させ、消毒薬で 0 ～ 4 分間処理した後、中和および定量的逆転写 PCR（RT-PCR）による定量化を行った。さらに、有効な消毒薬を有機物の負荷状況下で評価した。
結果
希釈塩素系漂白剤と次亜塩素酸塩ベースの市販消毒薬のみが、あらゆる種類の遊離核酸を有意に分解した。過酸化水素ベースおよび四級アンモニウムベースの消毒薬により、すべてのターゲットで 4 分後に 1 log 未満の減少が認められた。結果は、各ターゲットにおいて時間依存的であり、適切な接触時間の重要性を強調するものであった。有機負荷は、核酸分解における次亜塩素酸塩ベースの消毒薬の有効性にほとんど影響を及ぼさないと考えられる。
結論
本研究により、分子診断検査用サンプルを処理する場合、残存核酸を除去するための消毒薬の適切な選択および適用の重要性が明らかにされた。
監訳者コメント：
実験台やピペットなどの器具の表面への核酸汚染により発生する偽陽性は、核酸増幅検査において必ず回避すべき課題であり、検査診断の信頼性を揺るがすことになる。過去より次亜塩素酸ナトリウムが使用されてきているが十分な根拠がなかった現状があり、本研究により次亜塩素酸ナトリウムで適切な接触時間を確保できれば、環境表面の微生物の殺滅のみならず、遊離核酸を十分に分解すること、また有機物によっても分解効果に影響しないことが証明された。
