基質特異性拡張型β-ラクタマーゼ（ESBL）産生腸内細菌目細菌の保菌または感染の既往を有する患者に対する ESBL 産生腸内細菌目細菌の消化管保菌を対象とした再入院時の系統的スクリーニング★

背景
基質特異性拡張型β-ラクタマーゼ（ESBL）産生腸内細菌目細菌は、消化管にみられる共生多剤耐性菌であるが、その検出率はこの 20 年間で世界的に、また欧州において急激に増加している。
目的
5 年間の調査において ESBL 産生腸内細菌目細菌保菌が認められた患者 2,509 例の大規模コホートを対象に、再入院時に消化管保菌について評価すること。833 例（33％）が少なくとも 1 回再入院していた。
方法
フランスの 3 次病院 1 施設で実施する後向き単一施設調査。
結果
再入院を複数回（範囲2 ～ 13 回）した患者において、再入院時に ESBL 産生腸内細菌目細菌保菌を対象とした系統的スクリーニングの検出により、再度保菌が認められた患者の割合は、最初に ESBL 産生腸内細菌目細菌が認められた過去の入院から 18 か月以内では 80％を越えていた。
結論
長期間にわたり再入院時に再度保菌が認められる患者の率が高いため、ESBL 産生腸内細菌目細菌保菌を対象とした系統的スクリーニングの継続を再考する必要性がある。
監訳者コメント：
フランスの単一の大学病院からの報告。ESBL 産生菌検出歴のある患者の再入院時の保菌率が 90％と高いため、スクリーニング検査を継続するよりは原則として ESBL 産生菌保菌患者として対応することが妥当と考えられる。
