Compliance with a novel hand hygiene protocol tailored to non-sterile healthcare workers in the operating theatre

背景

手術室における未消毒の医療従事者について手指衛生遵守を観察することは、専用のプロトコールや観察ツールがないため困難である。

目的

手術室における未消毒の医療従事者を対象とした手指衛生プロトコールを開発して検証すること。

方法

本前向き観察研究において、Rotterdam-Rijnmond 地域にある病院 9 施設から、手術室における手指衛生に関するプロトコール草稿について情報を得た結果、同地域における新規プロトコールとしてコンセンサスを得た。同プロトコールに基づいた観察ツールを開発して検証した。病院の種類および医療従事者の職種ごとに、手指衛生遵守率と 95％信頼区間（CI）を算出した。

結果

本プロトコールは以下の 3 部から成っている：（1）手指衛生規則書、（2）麻酔・手術における手指衛生規則書、（3）手術室を 4 つの手指衛生区域に分けた図面。手指衛生は、区域が替わるときに実施することとした。全病院の手術室において、観察された手指衛生遵守率は平均で48.0％（95％CI 45.2 ～ 61.2％）であった。手指衛生遵守率は 専門病院 2 施設において最も高く（64.0％、95％CI 30.6 ～ 89.8％；76.7％、95％CI 62.8 ～ 84.5％）、学術教育病院で最も低かった（23.1％、95％CI 0.0 ～ 45.8％）。医療従事者の職種に関しては、手指衛生遵守率は麻酔科医で最も低く（31.6％、95％CI 19.2 ～ 62.4％）、手術室助手で最も高かった（57.4％、95％CI 50.1 ～ 78.2％）。

結論

手術室における手指衛生遵守に関するこの標準化された観察法により、手術室における介入の有効性の評価が可能になり、病院間の切磋琢磨が促進される。Rotterdam-Rijnmond 地域において、手術室の手指衛生遵守率は 50％未満であった。この状況、特に教育病院および医師について対処する必要がある。

WHO の手指衛生の 5 つの瞬間では患者ゾーンと医療領域との区別が重要であるが、手術室においてはこの区別が難しい。本研究はオランダで手術室での手指衛生のプロトコールを開発し、手指衛生区分を 4 つに分けて遵守率を評価したものである。
