Evaluating the environmental microbiota across four National Health Service hospitals within England

微生物汚染物、例えば乾燥表面バイオフィルムにより汚染された病院表面は、病原性微生物のリザーバの役割を果たすことがあり、その検出ならびにルーチンの洗浄時における除去を妨げることがある。諸研究から、そうしたバイオバーデンの増加が、消毒薬の効果を妨げ、潜在的な病原体の検出ができなくなる可能性が認識されている。医療環境内の清潔さは、ルーチンの培養に基づく分析によって判定されることが多く、表面で 2.5 コロニー形成単位（CFU）/cm² を超えることが示された場合に患者の健康にリスクをもたらす。したがって、過小評価することになれば、有害な影響をもたらしかねない。本研究では、19 か月にわたりイングランドの病院 4 施設の接触頻度が高い表面における微生物増殖を定量した。そのための方法として、患者の近接範囲内の様々な表面のサンプルを得るために環境スワブを用い、これらのサンプルを非特異的低栄養寒天培地で培養した。環境から物理的に除去された表面における乾燥表面バイオフィルムの存在は、落射型微分干渉顕微鏡と落射蛍光の組み合わせによるリアルタイム・イメージングを用いて確認した。検査した表面の約 3 分の 2 は清浄度の限界を超えていた（中央値 2,230 CFU/cm²）一方、BacLight LIVE/DEAD 染色によるイメージングを行った表面の 83％では微量のバイオフィルムが確認された。感染制御策の違い、例えば表面消毒薬や洗浄担当者の選択などは、観察された微生物のばらつきや結果として生じる患者リスクには影響を及ぼさなかった。この結果は、すべての病院洗浄における現行の基準の有効性には限界がある可能性を示しており、この限界を克服するためには、代表的な臨床データを用いてさらなる開発を進めることが必要である。
