背景

中心ライン関連血流感染症(CLABSI)のリスク因子のモデリングは一般的だが、まさに起ころうとしている CLABSI をリアルタイムで予測するモデルは不足している。

 

目的

その後の 24 時間で CLABSI を発症する可能性のある患者を特定する予測モデルを構築すること。

 

方法

2010 年 1 月から 2020 年 8 月の間に、ボストン小児病院の心臓集中治療室または心臓病棟に入院し、中心静脈カテーテル(CVC)が留置されていた全患者(細菌性心内膜炎の診断を受けた患者は除外)において、感染症の同定に関連する可能性のある変数を収集した。患者がその後の 24 時間で CLABSI を発症する可能性があるかどうかを予測するモデルを作成した。独立した検証セット(40%)で作動させたモデルの曲線下面積(AUC)、感度および偽陽性率に基づいて、モデルの性能を評価した。

 

結果

7,468 例の個別の患者に対応する 104,035 患者日および 139,662 ライン日を本分析の対象とした。陽性の血液培養は 399(0.38%)であり、黄色ブドウ球菌(Staphylococcus aureus)が最も多く見られた(感染症の 23%)。主要な予測因子には、感染症の前病歴、最大心拍数の増加、最高体温の上昇、C 反応性タンパク質の増加、静脈栄養への曝露、CVC クリアランスのためのアルテプラーゼの使用が含まれた。本モデルによって 25%の陽性培養が同定され、偽陽性率は 0.11%であった(AUC = 0.82)。

 

結論

機械学習モデルはまさに起ころうとしている CLABSI 患者の 25%を予測するのに使用でき、これらの予測が不正確であるのは 1.1/1,000 のみである。前向きに検証されたら、本ツールによって早期の治療または予防ができる可能性がある。

監訳者コメント

機械学習によって、24 時間以内に CLABSI が起こるかも知れない症例の 25%をリアルタイムで教えてくれるとのこと。一方で、検討する部署によって様々な条件が異なるので、多くの部署や病院にすぐに応用できるというわけでもなさそう。前向きの検証も含め、まだまだ試行段階といえそうだが、目指す方向性は大変に参考になる。
