Comparison of the cleaning and disinfecting efficacy of four washer-disinfectors for flexible endoscopes

4種類の洗浄・消毒器（AdaptaScope、ETD-2Plus、Innova-E3、LS-2000）を比較した。これらの洗浄・消毒器の洗浄効果および処理効果を、外観検査およびドイツの検査方式（EN ISO/TS 15883-5）による微生物の減少係数（RF）で判定した。検査用の部品を血液およびEnterococcus faeciumで汚染させた。洗浄剤3種類（Cidezyme GI、ETD Cleaner、Liquid 52）と消毒剤3種類（Cidex OPA-C、ETD Disinfectant、Liquid 44）を使用した。水のみによる洗浄効果の検査も実施した。Cidezyme GIの有効性は、AdaptaScopeとの使用でRF 7.0、LS-2000との使用でRF 8.4であった。Liquid 52の有効性は、LS-2000との使用でRF 7.0であった。水を使用した場合の洗浄効果はAdaptaScope（RF 2.1）とLS-2000（RF 1.2）との間に有意差が認められた。全処理後の微生物学的有効性は、Cidezyme GI/Cidex OPA-CによるAdaptaScope（RF 8.4）とLiquid 52/Liquid 44によるLS-2000（RF 9.2）との間にも有意差が認められた。Innova-E3では、全処理の終了時点でも検査用の部品は汚染されたままであり、RFは算出できなかった。ETD-2Plusでは、全処理終了後のRFは低値であった（3.7～7.5から1.8）。全処理による効果に差は認められたものの、AdaptaScopeとLS-2000では一貫して細菌の減少が認められた。内視鏡チャンネルを人工的に閉塞させた内視鏡の処理では、AdaptaScopeとLS-2000ではエラーメッセージが表れて処理が停止したのに対して、ETD-2PlusとInnova-E3ではエラーメッセージは表示されなかった。

内視鏡の洗浄・消毒の効果は消毒液と洗浄・消毒器機の組み合わせで大きく影響を受ける。こうした消費者テストのような効果を中立な立場で評価し、データを公開することにより適切な選択が可能になる。
