病院感染肺炎のサーベイランスは極めて大量のリソースを要するため、病院感染肺炎サーベイランスの代替法が早急に必要とされている。本研究では、ルーチンの退院時の国際疾病分類第 10 版(ICD-10)に基づく診断コードによる病院感染肺炎の発生率を、妥当性が検証された Hospitals in Europe Link for Infection Control through Surveillance(HELICS/IPSE)のサーベイランス定義の患者記録の手動による後向き再評価を用いた病院感染肺炎の発生率と比較した。HELICS による病院感染肺炎に対して、ICD による病院感染肺炎の陽性予測値は 0.35、感度は 0.59 であった。したがって、現在利用可能な ICD-10 に基づくルーチンの退院データでは、病院感染肺炎患者の信頼性のある同定ができない。

監訳者コメント:
院内感染のサーベイランスは、感染予防対策プログラムの成否の指標であり、その報告は必須事項となっている。しかしながら、欧州サーベイランスの定義にしたがった病院感染肺炎(HAP)のサーベイランスは時間と手間がかかるため、国際疾病分類コード(ICD-10)を利用した効率の良い低コストの HAP サーベイランスの可能性を研究したのが、この論文である。人工呼吸器関連肺炎(VAP)については、と ICD-10 との比較はあるものの陽性予測率(0.21 ~ 0.57)と感度(0.18 ~ 0.21)共に十分ではないことがわかっている。本論文の HAP においても両者は十分ではなく、HAP サーベイランスに ICD-10 は使えず、自動的に HAP を検出する別のシステムの開発が望まれる。
