英国におけるカンジダ・オーリス(Candida auris)に対するリアルタイム PCR によるサーベイランス実施の最初の経験:2 つの国際クレードの複数の導入の検出と患者転帰の改善★

背景
カンジダ・オーリス(Candida auris)は、伝播が速く、高い死亡率をもたらすとされている。2018 年 1 月から、ロンドンの教育病院 1 施設 で PCR ベースの新規サーベイランスプログラムを開始した。この実施の 2019 年 3 月までの結果を、同環境で認められた臨床、伝播および系統発生に関する特性とともに報告する。
方法
C. auris を対象としたリアルタイム PCR アッセイを開発して、妥当性を検証し、皮膚スワブおよび尿サンプルに直接実施した。環境スワブに対する PCR 検査も、緊急アウトブレイク制御対策の一環として行った。患者の臨床的リスク因子および転帰を確認した。保菌患者 9 例の周囲に24 時間設置した平板培地による培養で環境内拡散を評価し、次いで保菌患者 1 例の周囲で乱流の発生度が高い活動と低い活動中に空気サンプル採取を行った。Illumina HiSeq を用いてシークエンシングを実施し、高速ブートストラップ解析を用いて最大尤度系統樹を作成した。
結果
C. auris 保菌患者 21 例が特定された。保菌検出の所要時間は中央値で 141 時間(5.8 日)から約 24 時間に短縮され、迅速な感染制御予防策の実施が可能になった。平板培地により、保菌患者の周囲で 24 時間に 70 ~ 600 cfu/m2 が検出され、空気サンプル採取から乱流が発生する活動中に拡散することが示唆された。C. auris DNA は、環境スワブの 35.7%で検出された。高リスクであるにもかかわらず、侵襲性感染症を発症した患者はいなかった。同施設から得られた分離株のシークエンシング解析から、南アジア株の2件の侵入(クレードI)および南アフリカ株の 1 件の侵入(クレード III)が検出された。
結論
PCR により、迅速かつ拡張可能なスクリーニング法が可能になり、複数の侵入に遭遇する可能性のある臨床的リスクを低減する助けとなる。
監訳者コメント:
Candida auris は日本での報告は多くないが、もともとは 2009 年に日本から報告された新しい真菌である。薬剤耐性が強いこと、菌株の特定が困難であること、環境に定着しやすく消毒が困難であることなど、やっかいな特徴がいくつもある。C. auris は伝播時間が早い一方で、一般的な Candida 属より増殖が遅いため、本研究で扱われている PCR 検査のような迅速性に優れた検査で有用性が高い。特にアウトブレイク時には効果を発揮する。海外からの人の流れが再度増えることを考えると日本で C. auris のアウトブレイクがいつ起きてもおかしくない状況で、それに備えておく必要がある。
