Impact of direct hand hygiene observations and feedback on hand hygiene compliance among nurses and doctors in medical and surgical wards: an eight-year observational study

背景

手指衛生遵守の改善は、医療関連感染症(HAI)の予防において極めて重要である。

 

目的

観察およびフィードバックが外科および内科病棟の看護師および医師における手指衛生遵守に影響を及ぼすかどうか、ならびにこれらの活動が HAI の発生率に影響を及ぼすかどうかを明らかにすること。

 

方法

本縦断的観察研究では、2013 年 5 月から 2020 年 12 月にフィンランドの 3 次病院 1 施設の内科病棟 6 カ所および外科病棟 7 カ所において、手指衛生遵守について、また HAI の発生率について観察した。病院の手指衛生および HAI のモニタリングレジストリから、「手指衛生の 5 つの機会」について観察データを収集した。統計学的解析には、多変量ロジスティック回帰分析および ポアソン 回帰モデルを用いた。

 

結果

手指衛生モニタリングには、看護師における 24,614 件の観察および医師における 6,396 件の観察が含まれた。内科病棟では、手指衛生遵守率が 10.8%上昇して86.2% から 95.5%になり、HAI 発生率が 1,000 患者日あたり 15.9 から 13.5 に低下した(P < 0.0001)。外科病棟では、手指衛生遵守率が 32.7%上昇して 67.6% から 89.7%になり、HAI 発生率が 1,000 患者日あたり 13.7 から 12.0 に低下した(P < 0.0001)。全体的な手指衛生遵守率は、看護師(17.8%)および医師(65.8%)で有意に上昇した。手指衛生遵守率は、看護師の方が医師よりも高かった(内科病棟:オッズ比[OR]3.36、95%信頼区間[CI]2.90 ~ 3.90、P < 0.001;外科病棟:OR 9.85、95%CI: 8.97 ~ 10.8、P < 0.001)。

 

結論

手指衛生の直接観察およびフィードバックにより、8 年間の期間中に看護師および医師において手指衛生遵守率が有意に上昇した。同期間中に、HAI 発生率は内科および外科病棟の両方で有意に低下した。

監訳者コメント:

まず、フィンランドにおける手指衛生遵守率の高さに驚く。介入前に内科病棟はすでに 8 割を超えており、介入後には 95.5%と驚異的な遵守率である。WHO は5 つのタイミングとともに、アルコール成分が 70%の消毒剤を手の大きさにあわせて1.6 ~ 3.2 mL を手にとり 20 ~ 30 秒間両手で擦りあわせることを推奨しているが、なかなか遵守率ととともに確実な手指衛生手順ができない現状がある。一般的に遵守率が 60%を越えると HAI 率が減少するとされている。また患者への接触度合いは、看護師が 71%であるのに対して、医師は 10%であり、看護師の手指衛生の遵守率向上が医療施設内感染予防の重要なポイントとなる。本論文では、8 年間という長期間の継続した直接観察とフィードバックで、手指衛生遵守率の改善がみられ、結果的に HAI の発生率低下をもたらしている。
