背景

2022 年 2 月 2 日から 4 月 6 日にかけて、3,500 床の病院 1 施設の脳神経外科患者において、セラチア・マルセッセンス(Serratia marcescens)による手術部位感染症 9 件が診断された。

目的

S. marcescens による感染症の感染源を追跡して、アウトブレイクを迅速に終息させること、また将来の流行を予防すること。

方法

2022 年 2 月以降のすべての脳神経外科手術および S. marcescens が分離された培養についてレビューを行った。患者および環境感染源からサンプルを採取した。S. marcescens 分離株について、抗菌薬感受性試験によって特性解析を行った。全ゲノムシークエンシング(WGS)を用いて、遺伝的関連性を検討した。耐性遺伝子、病原性遺伝子、およびプラスミドレプリコンを同定した。

結果

S. marcescens が、患者の穿刺液、脳脊髄液、およびその他の分泌液から分離され、理髪店の理髪器具(レザーナイフ、スリッカースクレーパー、およびかみそりを含む)の培養でも分離された。合計で分離株 15 株が患者から回収され、8 株が理髪器具から回収された。すべての分離株が、同一の抗菌薬耐性パターンを示した。WGS により、分離株 23 株において密接なクラスタリングが示され、以前の分離株とは大幅に異なっていた。すべての分離株で 3 つの耐性遺伝子および 9 つの病原性関連遺伝子が検出され、23 株中 19 株が MOBP型 プラスミドを保有していた。これらの結果から、S. marcescens によるアウトブレイクであることが確認され、感染源は病院内の理髪店の汚染された理髪器具であることが明らかになった。今回のアウトブレイクは、感染制御策および理髪師の再教育の広範な強化後に終息した。

結論

これらの結果は、術前の皮膚前処置に関連する術後感染症のリスクがあることを示しており、アウトブレイク調査の促進における次世代シークエンシングツールの価値を実証している。

病院内理髪店の汚染された理髪器具が原因となったセラチア菌による脳神経外科 SSI のアウレブレイクの中国からの報告。SSI 予防のために頭髪を除去するのにカミソリでなく、バリカンを使用することの重要性がわかる。病院内理髪店など院外の業者が関与する場所では医療関連感染の管理上の盲点になりやすい。
