医療関連感染症に起因する入院期間延長を推定するための多状態モデル、生存回帰およびマッチングによる症例対照法の比較★

背景

最近のシステマティックレビューでは、医療関連感染症(HAI)の入院期間(LOS)延長を推定する場合、バイアスを最小化するための時間依存法が推奨された。しかし、どの時間依存法が HAI 発生率の研究への使用に最も適しているかに関するエビデンスは、ほとんど存在していない。

 

目的

国民保健サービス(NHS)スコットランドの教育病院 1 施設および総合病院 1 施設における、1 年間の前向き HAI 発生率研究のデータに関し、後向き分析を行うこと。

 

方法

3 つの時間依存法(多状態モデル、多変量補正生存回帰、マッチングによる症例対照アプローチ)をデータに適用し、LOS 延長の推定および比較を行った。

 

結果

多状態モデルから推定された未補正の LOS 延長は 7.8 日(95%信頼区間 5.7 日~ 9.9 日)であり、入院時特性を補正した生存回帰から推定された LOS 延長(9.9 日、8.4 日~ 11.7 日)およびマッチングによる症例対照アプローチによる補正推定値(10 日、8.5 日 ~ 11.5 日)よりも短かった。3 つの時間依存法による全推定値は、27 日間の時間固定粗推定値よりもはるかに小さかった。

 

結論

HAI に関連した LOS を検討する研究では、時間依存バイアスを最小限として対処するデザインを考慮すべきである。HAI 群と 非 HAI 群の間に患者特性の不均衡がある場合、患者特性の補正も重要であり、そこで時間依存共変数を伴う生存回帰が最も柔軟なアプローチをもたらす可能性が高い。マッチングによるデザインはデータ損失につながる可能性が高く、一方で多状態モデルは共変数の補正の問題によって制限がある。これらの結果は、感染予防・制御プログラムの費用対効果に関する今後の研究に対して、重要な意味を持つものである。

監訳者コメント

医療関連感染症の入院期間延長を推定する 3 つの手法について、それぞれ解析した論文である。手法によるバイアス、かかる時間、(場合により)費用などを合わせ検討する必要があると思われ、本論文の検討は大変興味深いものであった。
