背景

ドイツでは 2020 年 12 月に、医療従事者に対する重症急性呼吸器症候群コロナウイルス-2(SARS-CoV-2)のワクチン接種が開始された。病院には、ワクチン接種戦略に対する準備期間がほとんどなかった。

 

目的

ドイツの感染制御担当者から、医療従事者に対する初回ワクチン接種戦略に関する情報を収集すること。

 

方法

倫理委員会から承認を受けた横断的質問票をオンライン調査の形式で作成し、予備テストを行った。2021 年 3 月および 4 月に、無作為に選択したドイツの病院 987 施設において衛生/感染予防の責任を有する感染制御担当者に対して調査への参加を求めた。統計解析のために、病院を病床数に基づいて 2 群に分類した(小規模:500 床未満、大規模:500 床以上)。

 

結果

感染制御担当者 987 名中 100 名(10%)が本調査に回答した。参加病院の 80%で、医療従事者におけるワクチン接種の優先性決定は、ドイツ予防接種常設委員会(STIKO)の推奨に基づいていた。にもかかわらず、感染リスクのある患者と接触する医療従事者のワクチン接種が優先されると回答したのは 54%のみであり、したがって STIKO の推奨から逸脱が認められた。個人的に高いリスクを有する医療従事者については、24%の病院でワクチン接種が優先されると回答した。ワクチン未接種の医療従事者を感染リスクの低い区域に移動することを検討していたのは、大規模病院では 2%、小規模病院では 12%であった。

 

結論

ワクチン接種の優先性決定には病院によってばらつきがあり、STIKO の推奨からの逸脱が認められた。パンデミックに対する準備という考え方には、一般的なアプローチとは異なる戦略が及ぼし得る影響に取り組むことが求められる。さらに、今後の研究では医療従事者が未接種のままに留まる理由を分析することが、効果的な戦略を採用する上で必要である。このことは、医療施設レベルでのワクチン接種の義務化という背景を考えると、特に重要である。

監訳者コメント

新型コロナに対するワクチンの医療従事者への第 1 回目の接種開始前のアンケート調査であるが、回答率が 10%と低いものの、調査結果からは必ずしも国の方針(STIKO)通りにはゆかず、病院毎の方針で動いていることが明らかとなった。日本では医療従事者のワクチン接種は義務化されておらず、努力義務である。重篤なアレルギーや免疫抑制の状態の医療従事者を感染リスクの低い部署への異動も考慮すべき点であり、感染後の重症化を考えると妥当である。
