3 次病院でのカルバペネマーゼ産生カルバペネム耐性腸内細菌目細菌の曝露者における感染率の全ゲノムシークエンシングによる調査★

背景
カルバペネマーゼ産生カルバペネム耐性腸内細菌目細菌(carbapenemase-producing carbapenem-resistant Enterobacterales:CP-CRE)の伝播を減少させるために、CP-CRE 検出患者と同室の患者やこれらの患者をケアする医療従事者におけるスクリーニングが推奨される。
目的
本研究の目的は、3 次病院での CP-CRE 曝露者における感染率を、全ゲノムシークエンシングにより調査することである。
方法
本研究は、1,751 床の 3 次教育病院において2017 年 1 月から 2019 年 12 月まで実施した。入院期間に CP-CRE 検査陽性となった患者を初発患者と定義した。同室で初発患者が検出された場合、入院が初発患者と少なくとも 1 日重なった患者において CRE スクリーニング検査を実施した。第 2 の症例が確認された場合、医療従事者の接触もスクリーニングした。PCR 法により CP-CRE を確認し、カルバペネマーゼの型を同定した。全ゲノムシークエンシングを用いて、初発患者と曝露患者の分離株を比較した。
結果
研究期間中、初発患者 47 例が特定され、これらの患者は同室の患者 152 例、医療従事者 54 名と接触していた。CRE が検出された医療従事者はいなかった。曝露患者 152 例のうち 4 例で、CP-CRE 初発患者と同じ型のカルバペネマーゼが検出され、そのすべてが肺炎桿菌(Klebsiella pneumoniae)カルバペネマーゼ(KPC)であった。全ゲノムシークエンシングにより、これら 4 ペアのうち 3 ペアにおいて CP-CRE 初発患者と曝露患者間で遺伝子型の一致が認められた。
結論
曝露患者における CP-CRE 感染率は 2.0%(152 例中 3 例)であった。
監訳者コメント:
全ゲノムシークエンシング技術の発展に伴い、コストを安く、しかも早く結果がだせるようになってきた。様々なマーカー遺伝子をもとに比較も可能となり、分子疫学的解析も新時代の幕開けを迎えている。
