Economic evaluation of pre-operative shower with antiseptic soap to prevent surgical site infections

背景

手術部位感染症（SSI）を予防するために、手術前の術前シャワーが推奨されている。

方法

フランスの大学病院 1 施設において、SSI 発生率、ならびに術前に抗菌石けんを用いたシャワーを受ける患者における費用節減の可能性についてモデルを作製した。

結果

抗菌石けんを用いたシャワーにより SSI 209 件が予防され、年間 632,210 ユーロの節減が得られる可能性が示された。手術の種類別に分類した結果から、年間の費用節減は、整形外科手術で 26,537 ユーロ、産婦人科手術で 20,520 ユーロ、ならびに消化器手術で 14,377 ユーロであることが示された。

結論

抗菌石けんを用いた SSI 予防における有効性に関する発表データはないが、筆者らは、外科的介入前に抗菌石けんを用いたシャワーを普及させることで、費用節減およびベネフィットが得られる可能性を示した。

監訳者コメント：

CDC、日本手術医学会など複数のガイドラインで、手術前の入浴またはシャワー浴が推奨されている。ただし、皮膚に付着する細菌数を減らすことが、実際に SSI の低下に寄与するかどうかは明らかにされてこなかった。本論文では費用節減に焦点を絞っており、予防された SSI についての詳細は記載されていなかった。根拠の積み重ねが必要であると思われた。
