Hand hygiene behaviours monitored by an electronic system in the intensive care unit–a prospective observational study

背景

手指衛生遵守の改善は困難であり、医療従事者において基本的な行動が十分に理解されていない。

目的

医療従事者の手指衛生行動を調査すること。

方法

本研究は前向き観察研究である。手指衛生のための Sanibit 電子システムを 10 床の外科集中治療室（ICU）に設置した。このシステムは、医療従事者が病室に出入りする場合の手指衛生の機会を自動的に検知し、医療従事者の手指衛生遵守を追跡する。手指衛生遵守率と患者接触時間を ICU レベルと個人レベルの両方で経時的に計測し分析した。

結果

今回の 6 か月の試験期間中に、計 27,692 回の手指衛生の機会が記録された。医療従事者が退室する時の方が入室する時よりも手指衛生遵守率は有意に高かった（37.3％対 26.1％、P ＜ 0.001）。「入退室時間が短い」（病室内 3 秒未満［原文のまま記載。30 秒未満の誤記載と思われる］）場合の方が、「入退室時間が長い」（病室内 30 秒以上）場合よりも、手指衛生の完全遵守率、不完全遵守率、総遵守率が有意に低かった（それぞれ、23.45％対 32.77％、21.44％対 35.03％、44.88％対 67.81％、P ＜ 0.001）。また、個々の医療従事者間で手指衛生遵守に有意差が認められた（P ＜ 0 .001）。1 日の時間帯または 1 週間の各曜日において手指衛生の総遵守率および患者接触時間に有意差は確認されなかったが、手指衛生の不完全遵守率には有意差が確認された。

結論

医療従事者間の手指衛生行動パターンは複雑でばらつきがあるが、的を絞った個別の介入の推進により手指衛生遵守を改善することができるであろう。
