Bacterial contamination of bone allografts in the tissue banks: a systematic review and meta-analysis

同種移植骨は、無菌条件下で採取および移植される。しかし、これらの過程での移植片の細菌汚染リスクは、健康上の懸念の 1 つである。同種移植片の無菌性を確実にするため、バイオバーデン試験および細菌汚染の検出が実施されている。本研究は、異なる分類に基づいて、同種移植骨における細菌汚染の発生率を決定することを目的とした。本研究の PROSPERO 登録番号を受け取った。PubMed と EMBASE のデータベースにおいて、2000 年 1 月から 2021 年 3 月の間で、関連するキーワードを用いて系統的な検索を実施した。PRISMA フロー図に従って関連する研究を選択した後、データ解析に Stata ソフトウェアを使用した。I² ≧ 50％を研究間の異質性とみなした。17 件の研究の 19,805 個の同種移植骨において、細菌汚染の全発生率は 12.6％（95％信頼区間 0.100 ～ 0.152）であった。同種移植骨における細菌汚染率は、2010 年以前は 10.8％であり、2010 年 1 月から 2021 年 3 月までは 14.7％であった。アジア、ヨーロッパ、オーストラリアでの汚染頻度は、それぞれ 11.5％、14.3％、5.2％であった。骨の汚染率は、生体ドナー（7.5％）、移植時サンプリング（6.9％）および骨片培養物（6.3％）と比較して、死体ドナー（19.9％）、採取時サンプリング（13.5％）およびスワブサンプル（13.2％）において高かった。細菌汚染は、脛骨の 24.4％、腓骨の 19.7％、大腿骨の 13.2％、その他の骨の 21％から回収された。ブドウ球菌（Staphylococcus）属菌が骨由来の主要な分離細菌であり（分離された属の 63.2％）、プロピオニバクテリウム（Propionibacterium）属菌（10.6％）が続いた。結論として、同種移植骨の高汚染は健康上の懸念の 1 つであり、さらなる健康モニタリングおよび基準の改善の必要性が示されている。

監訳者コメント：

抄録にも繰り返し記載されているが、同種骨移植における細菌汚染は予後に大きく影響を及ぼすため、大変クリティカルな課題であると思われる。にもかかわらず、本レビューにより細菌汚染の割合の高さが明らかにされた。
