異なる酸素供給システムにおける COVID-19 患者による空気中の SARS-CoV-2 RNA 排出:前向き観察研究★★

背景
高流量式鼻カニュラ(HFNC)を含む様々な酸素療法の方法における COVID-19 患者による空気中の SARS-CoV-2 伝播のリスクに関する懸念は持続している。
目的
本研究の目的は、異なる酸素供給システムを使用している COVID-19 患者グループ間で、空気サンプル中の空中 RNA の存在を比較することであった。本研究では、空気サンプルにおける SARS-CoV-2 RNA 陽性に関連する因子の探索も行った。
結果
空気サンプルで SARS-CoV-2 RNA 陽性が認められたのは、HFNC使用患者 39 例中 3 例(8%)、マスク使用患者 13 例中 0 例(0%)であったのに対し、鼻カニュラ使用患者では 20 例中 5 例(25%)であった。空気サンプル陽性に関するオッズ比は、HFNC群では非HFNC群と比較して 0.52(95%信頼区間[CI]0.11 ~ 2.34)、鼻カニュラ群では非鼻カニュラ群と比較して 5.78(95%CI 1.24 ~ 27.01)であった。空気サンプル陽性であった患者は、空気サンプル陰性であった患者と比較して、症状発症後のより早期にサンプル採取が行われており(中央値 7 日対 10 日、P = 0.04)、診断用鼻咽頭サンプルの Ct 値がより低かった(中央値 22 対 26、P = 0.02)。
結論
空気サンプル陽性率は、酸素補助デバイスとは関連しておらず、ウイルス量と関連していた。これらのデータから、個人防護具の使用は、酸素補助デバイスではなくウイルス量に従ったリスク管理に基づくべきであることが示唆される。
監訳者コメント:
様々な酸素供給システムの中で高流量式鼻カニュラ(HFNC)は COVID-19 で、エアロゾル感染のリスクは高いとされている。本研究では空気中の SARS-CoV-2 RNA をサンプリングした結果、空気サンプル陽性率は酸素供給システムとは関連しておらず、COVID-19 患者のウイルス量と関連しており、個人防護具の選択はウイルス量に従うべきとしている。本研究はウイルス培養を行っていない、対象の多くがアルファ株である、ワクチン接種が普及していない段階での臨床研究という制約があるので、今後のさらなる検討が期待される。
