医療従事者および学生に対する敗血症に関する教育が学習および患者転帰に及ぼす影響：システマティックレビュー★

背景
敗血症は世界的に重要な医療上の問題であり、世界中の医療従事者が直面している主要な課題である。世界的な敗血症の負担を低減することを目的とした重要な一つの取り組みが、医療従事者に対して敗血症の早期発見および管理について教育するというものである。
目的
医療従事者および学生における敗血症の教育について包括的な評価を行うこと。
方法
6 つのデータベース（PubMed、CINAHL、Embase、MEDLINE、Cochrane Central Register of Controlled Trials、Scopus）の検索を行った。検索対象には、医療従事者および学生に対して実施された敗血症に関する何らかの形態の教育またはトレーニングについて記述・評価した研究を含めた。研究の転帰評価項目は、トレーニング評価のための改変カークパトリックモデルに従って要約した。
結果
本レビューに研究 32 件を組み入れた。学習内容について、Surviving Sepsis Campaign ガイドラインに従って報告した。研究 7 件が、敗血症の教育内容として医療従事者間のチームワークおよびコミュニケーションに関するトピックを含めていた。ほとんどの教育プログラムは効果的であり、知識に関する即時の転帰評価項目に対して正の影響を及ぼすことを報告していた。アクティブラーニング法、例えばシミュレーションやゲームベースの学習などを介して実施された介入は、講義による授業と比べて、全般的に効果が高かった。患者ケアプロセスおよび患者転帰における改善は、病院の敗血症ケアバンドルが存在していること、また同時に実施されていることと関連した。
結論
敗血症に関する教育介入にアクティブラーニング戦略を組み入れることで、学習者の長期成績が改善する可能性がある。さらに、敗血症に関する教育、ならびにプロトコールに基づく敗血症ケアバンドルは、ケアプロセスおよび患者の利益における改善を促し、相乗効果をもたらす可能性がある。
監訳者コメント：
システマティックレビューにより敗血症の教育介入が患者予後に影響を評価した研究。本研究に限らず、シミュレーションやゲームベースの学習などのアクティブラーニング法は教育効果が高いことが言われており、感染管理や他の領域でも活用できそうである。
