換気またはエアロゾル抽出:指向性空気清浄機、HEPA吸引装置、ならびに陰圧環境の有効性の比較★

背景
本研究は、隔離室の不足への対処にあたり、可能性のある代替案を検討するために、指向性空気清浄機、HEPA 吸引装置、ならびに標準的な陰圧室を備えた環境での、さまざまな呼吸活動(呼吸、頻呼吸、咳嗽、酸素療法)におけるエアロゾル曝露について評価した。
方法
非再呼吸式マスクおよび鼻カニューレを用いたさまざまな呼吸状態(正常、頻呼吸、咳嗽、覚醒)でエアロゾル曝露を測定した。本試験では、指向性空気清浄機、HEPA 吸引装置、標準的な陰圧室、これらの併用、ならびに非介入対照群を含む環境において、マネキンの頭部、体幹、足部のエアロゾルの速度と濃度を分析した。
結果
指向性空気清浄機、HEPA 吸引装置、標準的な陰圧室のすべての環境では、対照群と比較してエアロゾルの蓄積が低減した。とくに、呼気流量が増加する活動において、指向性空気清浄機、HEPA 吸引装置は、標準的な陰圧室の環境よりも一貫して効果的であった。非再呼吸式マスクまたは鼻カニューレを使用した場合、HEPA 吸引装置は、指向性空気清浄機よりも頭部でのエアロゾル濃度が高いことが示された。分時換気量が 10 L/分を超えた場合、指向性空気清浄機とHEPA 吸引装置はともに、標準的な陰圧室の環境よりもエアロゾルのクリアランスが良好であった。
結論
指向性空気清浄機と HEPA 吸引装置は、効果的なエアロゾルの低減をもたらし、パンデミック時に、標準的な陰圧隔離室の代替手段として有用性が示唆される。標準的な陰圧環境では、空間が連続的に換気されるが、指向性空気清浄機と HEPA 吸引装置は、エアロゾルの早期除去においてより効果的であり、蓄積を制御する。とはいえ、酸素療法中のエアロゾルの多方向への拡散に対して、HEPA 吸引装置の単一方向吸引は、指向性空気清浄機がもたらす広範囲の適用とは異なり課題もある。
監訳者コメント :
陰圧室の代替法として指向性空気清浄機と HEPA 吸引装置の使用はエアロゾルの低減効果を認めた。
