A strict mask policy for hospital staff effectively prevents nosocomial influenza infections and mortality: monocentric data from five consecutive influenza seasons

背景

病院内で獲得されたインフルエンザ感染症による死亡には、特にリスク因子を有する高齢患者において増加がみられている。しかし、高リスク患者および医療従事者ではともにワクチン接種率が低い。

目的

インフルエンザシーズン中に病院においてより効果的にインフルエンザ感染症を予防するために、感染が発生した病棟において全スタッフに対して全シフトを通じて、介入として厳格な「鼻腔および口腔粘膜保護策」を求める要件を導入し、その院内感染症発生率に対する影響を検討した。

方法

本研究のデータは、2015 年から 2019 年にわたる連続した 4 つのインフルエンザシーズンを対象期間とした後向き単一施設分析から得た。全スタッフを対象とした全シフトにおける介入として「鼻腔および口腔粘膜保護策」を、2017 年およびその後の流行シーズンに、3 例以上のインフルエンザ感染患者が発生した病棟で同時に導入した。入院しているインフルエンザ感染症患者における介入前後のデータを、院内感染症の発生率および死亡率について比較した。

結果

厳格な「鼻腔および口腔粘膜保護策」の義務化を行った期間（2017 年から 2019 年）中に、インフルエンザ感染症の院内発生率はほぼ 50％低下し（オッズ比［OR］0.40、95％信頼区間［CI］0.28 ～ 0.56、P ＜ 0.001）、これに伴って院内死亡率は 85％と大幅に低下した（OR 0.15、95％CI 0.02 ～ 0.70、P ＝ 0.007）。インフルエンザ感染症発生率およびリスク患者日から示される感染圧は介入前後で同等であり、看護師におけるワクチン接種率の低さも介入前後で同等であった。

結論

医療従事者に対する「鼻腔および口腔粘膜保護策」の義務化により、患者は院内インフルエンザ感染症およびそれによる死亡から効果的に保護された。

監訳者コメント：

本報告は流行時には勤務時のマスク着用遵守によりインフルエンザの院内での発生を防げることが後方視的研究により明らかとなった。インフルエンザあるいは現在流行中の新型コロナウイルス感染症を含む呼吸器感染症は、飛沫により口腔あるいは鼻粘膜を介した感染経路が主体であることがわかっており、マスクの重要性を再認識した結果となった。
