手術室における空気汚染を低減するためのクリーンエアスーツの能力を判定する指標としての拡散強度★

背景
股関節および膝関節全置換術後の手術部位感染症は、手術室の空気の質と関連付けられている。気密性がある密閉型スーツの着用、効果的な換気、および正確な作業方法を適用するというのは、手術室における細菌濃度を低く保つうえで鍵となる考え方である。欧州規格 EN 13795-2:2019 で指定されている乾燥条件での菌浸透試験は、手術用ガウンに用いられる材質に対するスクリーニング法である。活動している人からの細菌含有粒子の拡散と定義される拡散強度の測定は、被服システムの機能検査であり、拡散微粒子測定用チャンバー(以下拡散チャンバー)および手術室で計算される。両方の試験の結果は、手術用被服システムの比較に用いることができる。
目的
本研究では、スウェーデンで市販されている 5 種類の手術用被服システムについて乾燥条件での菌浸透試験の結果と拡散強度測定値との関連を検討する。
方法
これらの製品の機能について実験データを報告する。データは、拡散チャンバーおよび整形外科手術時の手術室における結果から算出した拡散強度として示す。
結果
乾燥条件での菌浸透 50 コロニー形成単位(cfu)以下を基準として試験を行ったすべての材料において、拡散強度の値は拡散チャンバー内では l 人当たり 3 cfu/秒未満であった一方で、1 製品の材料は 50 回超の洗浄を行った場合の拡散チャンバー内での拡散強度が最大 8 cfu/秒であった。
結論
乾燥条件での菌浸透試験によって同じデザインのクリーンエアスーツの性能は予測できるが、より妥当性の高い相関関係を知るためにはさらなる研究が必要である。製品規格の要件には、拡散強度を含めるべきである。
監訳者コメント:
整形外科領域での股関節や膝関節の置換手術数は高齢者の増加と共に増加傾向にあるが、術後の創部感染症は医療費増大を招くのみならず患者の予後にも影響する。術後の深部感染症は手術室内の空気中に浮遊する細菌によると考えられており、術野に混入することにより発生すると考えられている。したがって、手術室内の空気の清浄度を保つために皮膚落屑に付着する細菌の飛散を防ぐ密閉型の手術用クリーンエアスーツが必要となる。本論文では 5 つのクリーンエアスーツの性能を欧州規格 EN13795-2:2019 の標準規格にはないが、その追加情報として掲載されている拡散強度による評価を実施している。
