背景

非アウトブレイク時の医療従事者、患者、病院環境へのメチシリン耐性黄色ブドウ球菌(Meticillin-resistant Staphylococcus aureus;MRSA)の定着が、MRSA 伝播に及ぼす作用は十分に理解されていない。

 

目的

MRSA 流行歴がある病院において、非アウトブレイク時の医療従事者、患者、環境に影響を及ぼす伝播イベントについて全ゲノムシークエンシング(WGS)により検討すること。

 

方法

9 病棟の医療従事者(326 名)と患者(388 例)のボランティアを対象に、鼻腔内および口腔内の MRSA 保菌について 2 年間にわたり検査した。患者が近接する環境(1,164 個)、接触頻度の高い場所(810 個)、空中(445 個)のサンプルについて、MRSA スクリーニングを実施した。MRSA 代表株および臨床分離株を、WGS、コアゲノム multilocus sequence typing(cgMLST)により分析した。近縁株(対立遺伝子の差異 24 以下)を類縁株群(RIG)に分類した。

 

結果

全体で MRSA 155 株を回収し、内訳は臨床分離株(41 株)、医療従事者(22 株)、患者(37 株)、環境分離株(55 株)であった。配列決定された MRSA 155 株のうち 110 株で 9 種類の clonal complex(CC)が同定され、110 株のうち 77 株がCC22 に分類された。MRSA 79株が 17 種の RIG に分類された。多数の潜在的な伝播イベントが、CC22-MRSA(RIG 1 ~15)、CC45-MRSA(RIG-16)、CC8-MRSA(RIG-17)と関連した。9 病棟のうち 7 病棟で 17 か月間にわたり、医療従事者6 名、患者 6 例、臨床サンプル 4 個、環境サンプル 8 個から 24 株の ST22-MRSA-IVh 型 を含む RIG-1(最多の RIG)が回収された。医療従事者/患者の分離株による医療従事者から患者、医療従事者間、患者間、および環境の汚染に関係する伝播イベントが明らかになった。医療従事者、患者、臨床、環境からの分離株は、それぞれ 4 種の RIG、9 種の RIG、7 種の RIG、13 種の RIGにおいて同定され、この 13 種中 12 種は医療従事者および/または患者の分離株の近縁株であった。

 

結論

非アウトブレイク時の医療従事者、患者、環境に影響を及ぼす多数の潜在的な MRSA 院内伝播イベントが、WGS により認められた。

監訳者コメント

黄色ブドウ球菌は常在菌ではないが、健常人でも鼻腔等の通過菌として一次保菌することがある。易感染性患者やアトピー性皮膚炎など皮膚疾患があると黄色ブドウ球菌は定着しやすくなる。その黄色ブドウ球菌が MRSA であった場合は、難治性になる可能性がある。患者療養環境の保菌圧を低減することは、施設内伝播の抑制につながり予防的に医療関連感染を防ぐことに繋がる。
