背景

シンクおよび排水管関連のカルバペネマーゼ産生腸内細菌目細菌(carbapenemase-producing Enterobacterales)の伝播は以前に報告されているが、基質特異性拡張型β– ラクタマーゼ(ESBL)産生腸内細菌目細菌の伝播に関する研究は限られている。

 

目的

一般外科病棟 1 棟の患者用シンクおよび排水管を介した ESBL 産生肺炎桿菌(Klebsiella pneumoniae)の院内伝播について調査すること。

 

方法

2019 年 7 月から 2020 年 2 月に、東京医科大学病院の外科病棟 1 棟(2019 年 7 月に建設)における ESBL 産生肺炎桿菌の伝播について調査した。全ゲノムシークエンス解析によって、患者および環境由来の分離株の関連性に関する情報を得た。

 

結果

本研究中に、肺炎桿菌の臨床分離株 4 株(TUM19831、TUM19832、TUM19833、TUM19834)が検出された。TUM19831 は新しい建物に移転する前に同定されており、新しい建物でも再び検出された。TUM19832 と TUM19833 は 2019 年 7 月に検出され、TUM19834 は 2019 年 12 月に検出された。TUM19835 と TUM19836 は 2019 年 7 月に 2 つの異なるシンクと排水管で検出され、その上、2020 年 2 月にはさらに 2 つのシンクと排水管が TUM19837 および TUM19838 に対して検査陽性となった。全ゲノム解析によって、すべての菌株の配列型は ST307 および CTXM15 であり、分離株は遺伝子解析では識別不能であることが明らかになった。粘液バイオフィルム膜の除去が不十分であったため、TUM19837 および TUM19838 を検出する前にシンクを再び清掃する必要があった。

 

結論

本研究によって、一般外科病棟 1 棟の患者区域のシンクおよび排水管を由来とする、識別不能な ESBL 産生肺炎桿菌株の伝播が実証された。このリスクを認識し、病院およびその他の医療現場における配管システムの最適な管理戦略を開発する必要がある。
