がん患者におけるカテーテル関連血流感染症:左側挿入と右側挿入の比較★

背景
がん患者を対象に、中心静脈カテーテル(CVAD)の挿入する側と血流感染症リスクとの関連を検討した研究は少ない。
目的
右側挿入と左側挿入について感染症発生率を比較する目的で、無作為化対照試験(RCT)のデータおよび前向きコホート研究のデータを用いて探索的解析を実施した。
方法
本研究の対象集団は、14 歳超のがん患者とし、オーストラリア・ブリスベンの 3 次病院 2 施設で登録した。主要転帰は、盲検化された評価者が判定したカテーテル関連血流感染症(CABSI)とした。RCT では、Cox 比例ハザード回帰を用いて、早期(14 日以内)と後期(15 日以降)における左側挿入と右側挿入について、無作為化したintention-to-treat比較を行った。RCT のデータについては、RCT 実施前に病院の 1 つで収集したコホート研究データとも統合し、左側挿入と右側挿入について無作為化によらない比較を行った。
結果
無作為に割り付けられた CVAD 634 個において、CABSI 141 件が発生した。解析の結果、右側挿入群では早期感染症のリスクが 2.5 倍高い(ハザード比[HR]2.48、95%信頼区間[CI]1.3 ~ 4.7)という強いエビデンスが示されたが、後期感染症について有意なリスク上昇のエビデンスはなかった(HR1.06、95%CI 0.71 ~ 1.59)。RCT とコホート研究の統合データ(CVAD 2,786個 における CABSI 385 件)による解析では、早期感染症と後期感染症の比較は同様の結果であった。
結論
ライン挿入後約 2 週間にわたり、がん患者における CABSI のリスクは、右側に挿入された CVAD についてより高いようである。このリスク上昇の基礎にある機序については不明である。
監訳者コメント:
トンネル型の中心静脈ラインの挿入において、右側からの挿入が左側に比較して挿入 2 週間以内での CABSI が高い理由としては、多くの患者がが右利きであり、右手を主に動かすためにカテーテルのわずかな動きが生じ、これが細菌の侵入を助長する結果おこるのではないかと当初推測されたが、その後の追試で否定された。解剖学的な左右差、挿入時のやりにくさが左右で異なること、挿入者の利き手による違いなどが考えられたが、現時点では明確な答えは引き出せていない。
